Dr. Amber Farook, MD, is a family medicine physician who offers primary care services to patients of all ethnicities and ages. Dr. Farook and the team provide various services, including chronic disease management, pediatric care, women's health, joint injections, medical weight loss, and mental health evaluations.
Grace Chao, PA-C is a certified physician assistant and welcomes new patients of all ages. She provides primary care services including preventative/wellness exams, chronic disease management, mental health, and has a passion for women's health and the pediatrics/adolescent population.
The team’s goal is to provide each patient with a family practice setting that allows them to discuss health concerns and get customized treatments for medical conditions and aesthetic problems.
