Sliding Fee Scale

LaSante Health Center provides quality medical and dental services to everyone, regardless of a patient’s ability to pay. We accept Medicare, Medicaid and various commercial insurance plans. Insured and uninsured patients may be eligible for discounted rates on a sliding fee scale, based on annual income and family size. Staff are available to assist patients in determining eligibility for participating in the sliding fee scale discount program.
