Sliding Fee Scale

LaSante Health Center provides quality medical and dental services to everyone, regardless of a patient’s ability to pay. We accept Medicare, Medicaid and various commercial insurance plans. Insured and uninsured patients may be eligible for discounted rates on a sliding fee scale, based on annual income and family size. Staff are available to assist patients in determining eligibility for participating in the sliding fee scale discount program. Most importantly, no one is denied care due to lack of insurance or inability to pay for services.

Dr. Farha Zibak received her Doctor of Dental Surgery from NYU College of Dentistry, after which she completed her General Dentistry residency at Kingsbrook Jewish Medical Center.

Dr. Zibak was born and raised in Brooklyn. Her passion for dentistry started when she pursued her bachelor's degree in biology at Stern College for Women.
