Fondly referred to as “Dr. Chuck” by patients and colleagues, Chuck Osuagwu, MD, provides comprehensive internal medicine care to patients at Centerville Medical Center. At the office in Garland, Texas, Dr. Osuagwu is passionate about finding and correcting the root cause of illness, not just the symptoms. He is board certified in internal medicine and has been in practice for over 10 years.

Dr. Osuagwu attended medical school at UT Southwestern Medical Center in Dallas, where he was awarded numerous honors. He later completed his internal medicine residency at UT Southwestern working at Parkland Memorial Hospital, Dallas VA hospital, and Zale Lipshy University Hospital.

I earned my bachelor’s degree in Microbiology from the University of California, Davis. I graduated from Creighton Medical School in Omaha, Nebraska and completed my residency in Internal Medicine and Pediatrics at Louisiana State University - at Charity Hospital, in New Orleans. For five years, I was Chief of Urgent Care at Kaiser Permenente for the Dallas Region. Since 1997, I have been an attending Physician at Community Oriented Primary Care (COPC) in Garland, TX - A Division of Parkland Health and Hospital System. Currently, I am an attending physician at Children’s Hospital. In addition, I have hospital privileges at Richardson’s Regional Medical Center. When I am not in my office, I am with my two children who give me deep joy and happiness. We currently live in Richardson Texas.

Our clinic is led by Henna Xiang, MS, APRN, NP-C. Henna has been working as a Registered Nurse and Nurse Practitioner for over 25 years in the hospitals, home health, inpatient rehabilitation hospitals, health insurance company, and primary care clinic. She serves as a Graduate Nursing Clinical Education Specialist at University of Arlington. She also founded a non-for-profit organization “International Health Education Alliance” to promote health and education around the world. Henna is a proud mother of three wonderful children. Henna is actively involved in her church and her community. In her spare time, she enjoys traveling with her husband, photography, and flower arrangement. Raj Kakar, MD, is a snoring and sleep apnea specialist at Dallas Sleep, with locations in Plano and McKinney, Texas, who absolutely loves his work. His passion for helping others get the treatment they need to improve their sleep health and well-being is documented by his dedication to his patients and the DFW community. Since 2006, Dr. Kakar has been managing Dallas Sleep, also known as the Dallas Center for Sleep Disorders. Harman P. Kaur, MD is a Board-Certified Physician who practices Primary Care, Internal Medicine, Hematology, and Medical Oncology at Belt Line Medical Clinic for patients in Mesquite, Garland, Forney, Sunnyvale, and the surrounding areas in the DFW Metroplex. She provides individualized care for all common medical conditions including specialized treatment for diabetes, hypertension, cancer prevention, allergies, asthma, obesity, high cholesterol, anemia, thyroid disorders, COPD, and sleep apnea. She understands that not all patients are the same, and customizes solutions to health problems that are specific to each person's overall social, and physical needs.

Dr. Kaur has worked at various hospitals and clinics in the Dallas-Fort Worth area including Baylor Hospitals, Texas Regional Medical Center, and the Cancer Institute of Dallas in Duncanville, Texas. She opened Belt Line Medical Clinic in an effort to provide personalized health care that is rooted in dignity, compassion, respect, and quality for every patient. Dr. Harman P. Kaur has trained at some of the best training programs in the country. Before coming to the US, she finished medical school at Lady Hardinge Medical College in New Delhi, India where she graduated in the Top 10% of her class. She completed her internship at the Internal Medicine Residency Program in Spokane, Washington (an affiliate of the University of Washington School of Medicine in Seattle, WA) and then her Residency at SUNY-Buffalo Internal Medicine Residency Program in Buffalo, NY. Her Fellowship Training in Oncology was at the nationally - acclaimed Wilmot Cancer Institute in Rochester, NY. This experienced was followed with a Faculty Position at Roswell Park Cancer Center in Buffalo, NY before moving to the Dallas area with her family. She has also co-authored various book chapters in medical textbooks which are used in teaching programs across the country.

Dr. Kaur is fluent in English, Hindi, and Punjabi. In her free time, Dr. Kaur enjoys spending time with her family, traveling, and volunteering time at the local Sikh Temple where she focuses on her faith, Sikhism. 

As a childhood asthmatic, I spent many hours in emergency rooms and doctors' offices. I witnessed firsthand the fragmented care faced by uninsured and underinsured patients in my diverse community. My parents’ faith inspired them to work for a better future for me and our community, instilling in me a lens of hope and servant leadership. I studied comparative biology at Tulane University and pursued neurobiology at Yale. After graduate school, I attended Harvard Medical School and completed a residency in internal medicine at Massachusetts General Hospital. I view medicine as my calling; primary care is my passion because it combines restoration, education, counseling, and advocacy. For over 30 years, I have been dedicated to public health in communities worldwide, including West Africa and Haiti. In the last 19 years, I have worked as a board-certified internist in Dallas and Austin, focusing on reducing health disparities related to COVID-19, hypertension, diabetes, and HIV.

Shahnaz Begum, MD, FACC, is a Board-certified cardiovascular disease specialist. She is an experienced physician who has been practicing cardiology for over 15 years. 

Dr. Begum received her medical degree from State University of New York (SUNY) Downstate Medical College in New York. She subsequently completed 3 years of training in Internal Medicine followed by another 3 years of specialized training in Cardiovascular disease/cardiology. She then worked at SUNY Downstate as a faculty physician and Assistant Professor of Cardiology until she moved to Dallas. After that she worked at Texas Health Resource and Plano Presbyterian Hospital for several years as an attending Cardiologist. Since then, she has been providing outstanding cardiac care at Heart Care located in Frisco, Texas. 

Dr. Begum has exceptional knowledge and skills. She excels in diagnosing and treating heart conditions including heart failure, coronary artery disease (CAD), valvular heart disease, arrhythmias, and many more. She also believes in prevention and therefore focuses on hypertension, cholesterol and multiple other conditions that can lead to heart diseases.  

In addition to assuring that her patients benefit from cutting-edge new technology, Dr. Begum is compassionate and values their time. Uchenna Ozuah, MD, is a compassionate and patient-focused caregiver. He is very caring, humble, highly respectful, and a good listener. He is proud to provide internal medicine services to patients throughout Aubrey, Texas, and the surrounding area at Zidon Internal Medicine.

 

A physician with over 11 years of experience in hospital medicine, Dr. Ozuah approaches care as a partnership with his patients and their families. He utilizes a holistic approach in order to achieve the best outcomes possible.

 

Dr. Ozuah graduated from Ahmadu Bello University in Zaria, Nigeria, in 1992. His initial post-graduate training was in OB/GYN before he moved to the US. He completed his residency training in Internal Medicine and a fellowship at Metropolitan Hospital Center (New York Medical College). 

 

Dr. Ozuah is board-certified in Internal Medicine, Hospice, and Palliative Medicine. At Zidon Internal Medicine, his top priority is to consistently provide an enhanced and highly fulfilling patient experience. He also specializes in weight management and immigration physicals. 

 

Dr. Ozuah is happily married to his wife, and they have three lovely children. He loves to learn, travel, and spend lots of time with his family and friends.

Sunil Modi, MD of Express Family Clinic is board-certified in both family medicine and geriatrics, which means he’s with you every step of the way as you journey through life. His goal is to provide primary care to people in Allen, Texas and the surrounding communities. His practice is personal, experienced, up-to-date, and, above all, comprehensive.

Dr. Modi earned his medical and surgical degrees at B.J. Medical College in Ahmedabad, India. After graduating, Dr. Modi came to the United States and completed his residency at Temple University/Conemaugh Valley Memorial Hospital in Pennsylvania in family practice. He then completed his residency in geriatric medicine at the University of Pittsburgh Medical Center.

Not only is Dr. Modi certified by the American Board of Family Medicine and the American Geriatrics Society, but he also holds memberships in the American Medical Association, the American Academy of Family Physicians, and the Texas Medical Association.

Throughout his education and medical career, Dr. Modi has been recognized for his customer service and awarded Best Resident and Exceptional Role Model for his residency. Dr. Modi, who speaks English, Hindi, and Gujarati, lives in Allen with his wife and two boys. In his spare time, he enjoys listening to music, reading, and practicing yoga.

Pradeep Parihar, MD has devoted his medical career to family medicine, with a subspecialty in sports medicine, to help patients at Express Family Clinic in Allen, Texas, lead long and active lives. With a relationship-first approach, Dr. Parihar works with his patients, both young and old, to monitor their health through holistic and preventive practices that keep them functioning at their very best during every stage of life.

Dr. Parihar earned his medical and surgical degrees at Shyam Shah Medical College in India and then traveled to the United States, where he completed his residency in family practice at East Tennessee State University in Johnson City, Tennessee. Not stopping there, Dr. Parihar received a fellowship in primary care sports medicine from St. Vincent Medical Center in Los Angeles.

To stay abreast of the best medical practices, Dr. Parihar became certified by the American Board of Family Medicine, and he holds memberships in the American Academy of Family Physicians, the Texas Medical Association, and the Association of American Indian Physicians.

Katherine, a family nurse practitioner, has over 10 years of health care experience. She is dedicated to providing exceptional care to every patient.

Katherine received her Bachelor of Science in Nursing with a minor in psychology in 2012 at Texas Woman’s University in Denton, Texas. She continued her education at Walden University in Minneapolis, Minnesota, where she received her family nurse practitioner credential and Master of Science in Nursing. Throughout her career, Katherine has cared for children with autism and spent many years working as a registered nurse in intensive care and interventional radiology.

With this experience, Katherine founded Grace Vitale Wellness, offering urgent care, chronic disease management, vaccines, minor procedures, drug screenings, and acute care services. The practice also provides an infusion center, physicals, obesity treatment, mental health care, hearing and vision screenings, and in-house lab testing. Grace Vitale Wellness values hands-on, highly interactive care, and Katherine considers her clients to be community partners and neighbors. 

Board-certified allergist-immunologist Thomas Fowler, MD, invests in cutting-edge technology continuing education to offer individualized, evidence-based care. Dr. Fowler completed his undergraduate studies at Brigham Young University in Provo, Utah, where he met his wife. He later earned his medical degree at UT Southwestern Medical School in Dallas, Texas, before heading to Creighton University in Omaha, Nebraska, for an internal medicine residency. After finishing his residency, Dr. Fowler completed an allergy-immunology fellowship at Louisiana State University in Shreveport, Louisiana.

 

He has published multiple papers on various allergy and asthma topics, including rare conditions such as cereal anaphylaxis.

 

Dr. Fowler works with each patient to develop individualized care plans. In addition to medication, injectables, and immunotherapy, he helps patients avoid allergy and asthma triggers while preparing for emergency situations. Prior to specializing in urgent care, he practiced family medicine for nine years in the US Navy at Camp Pendleton, CA and private practice on an Indian reservation in Temecula, CA.
