I am Dr. Carmen E. Gota, MD, an experienced internal medicine, rheumatology, and weight-loss specialist and the founder of SlimWell. I am double board-certified by the American Board of Internal Medicine and Rheumatology and care for adults aged 18 and older.

I was born in Transylvania, Romania. I completed my medical training at Iuliu Hatieganu University of Medicine and Pharmacy in Cluj-Napoca, România, where I graduated as a valedictorian. I did an Internal Medicine Residency, then joined the University staff as an Assistant Professor of Internal Medicine. In that capacity, I taught medical students the discipline of semiology and how to recognize the signs and symptoms of diseases.

I completed my US Internal Medicine residency at Lenox Hill Hospital on the Upper East Side of Manhattan in New York City. There, I learned how different the American healthcare system was from the one I just left. I also noticed that being curious, kind, and compassionate matters the same no matter where you practice medicine. 

Throughout all these years of practice, I struggled with the frustrations related to the limits of our care. I wondered in many instances about the wisdom of treating conditions without treating the root cause – the disease of obesity. So, I decided that I needed to learn more about Obesity Medicine.

I always believed and tried to help my patients comprehensively. As obesity is the root cause of many conditions, I realized that managing weight could be the key to more effective comprehensive health care for a lot of my patients. 

SlimWell is the product of this vision for a healthcare model that delivers comprehensive integrative care by targeting weight loss with the goal of major downstream health benefits. 

On a personal level, I believe my relationship with my patients is a partnership where we work together towards improved health.  

By practicing evidence based medicine, active listening, careful review and paying attention to detail, I assist patients in maximizing their health and quality of life. My goal is to empower patients to take control of their health, by equipping them with the knowledge necessary to keep their health conditions controlled, trouble shooting issues that are keeping them from living their best life, and providing options that may not be available everywhere or offered by their previous healthcare providers. My role is to listen, evaluate, and inform by providing expert opinion, facts options. I then create plans that are customized for each patients needs, limitations, and treatment goals. By providing access to professional medical care and support without the traditional time consuming office visits for non emergency medical issues allows patients to put their health first in today's fast paced world. Good health is priceless; I aim to make it easier to make healthcare a priority.

Razieh Mohseni, MD, is a Board Certified Vein Specialist and Board Certified Wound Specialist at Vein Clinics of Lake County, and Wound Care Center in Concord, Madison, and Middlefield, Ohio.

Dr. Mohseni received her medical degree and completed her residency in internal medicine at the University of Florence, Italy. She then came to the United States in 1996 as a cardiology fellow at University Hospitals in Cleveland, Ohio. She completed her residency in Internal Medicine and a fellowship in Geriatrics at University Hospitals of Cleveland as well.

After working as an Internal Medicine physician for 15 years, Dr. Mohseni began her training in venous disease in 2011, training with Dr. Carl Dando and Dr. Roger Hogue at Circulatory Center in Pittsburgh and Hogue Vein Institute in Minneapolis. Since then, Dr. Mohseni has performed thousands of endovenous ablations and sclerotherapy, as well as ambulatory phlebectomies.

Dr. Mohseni is a Diplomate of the American Board of Internal Medicine. She is also a Diplomate of the American Board of Venous and Lymphatic Medicine as well as a Diplomate of the American Board of Wound Management. Dr. Mohseni is additionally certified by the American Registry of Diagnostic Medical Sonography as a Registered Physician in Vascular Interpretation (RPVI) and a Registered Vascular Technician (RVT).

Dr. Mohseni is one of the few physicians in the country to hold all these credentials.
