With his advanced training in primary care and urgent care, Josh Shajan, MD, is the go-to provider for patients and their families in and around Greenwich, Stamford and areas. He cares for patients at Greenwich Primary Care in the Cos Cob neighborhood of Greenwich, and serves as Medical Director of the practice. 

After attending Bergen Catholic High School, Dr. Shajan earned admission to an 8-year BS/MBA/MD program, completing his bachelor’s degree at Union College in Schenectady, New York, his Master of Business Administration through Union Graduate College, and then received his Doctor of Medicine from Albany Medical College in New York in 2012.

Dr. Shajan then completed his residency in internal medicine through the Yale University School of Medicine at Greenwich Hospital in Connecticut.

Since completing residency, Dr. Shajan has provided comprehensive primary and urgent care services continuously in Greenwich. Additionally, he has also previously served as the medical director for a network of New York City-based urgent cares, prior to joining a private practice in Greenwich in 2018. WIth the launch of Greenwich Primary Care, Dr. Shajan continues his tenure of providing high-quality care comprehensive primary and immediate care for numerous Greenwich and surrounding-area residents and their families.

When he’s not busy caring for patients, Dr. Shajan enjoys spending time with his wife Kavya, and their son at their home in Greenwich.

Dr. Mansourati is a board-certified internal medicine physician widely recognized for his compassionate, evidence-based, and patient-centered approach to care. He earned his medical degree from the University of Kalamoon and went on to join Duke University as a Postdoctoral Researcher, where he studied cell biology and the cancer microenvironment. His passion for advancing medical science continued at the University of Southern California, where he contributed to research on innovative cancer therapeutics.

Dr. Mansourati completed his internal medicine residency at MedStar Health in Baltimore, gaining broad experience in managing complex medical conditions and delivering high-acuity urgent care. During this time, he also collaborated with Johns Hopkins University on cutting-edge research projects.

His dedication to advancing clinical care and research led to his selection for the competitive Hematology and Oncology fellowship at St. Joseph's University Medical Center. While he continues to train in this specialized field, Dr. Mansourati remains deeply connected to his roots in internal medicine and is proud to bring his diverse expertise to the patients of Greenwich Primary Clinic.

Outside of his medical career, Dr. Mansourati leads an active lifestyle and enjoys mountain biking, hiking, CrossFit, and playing basketball.

Melanie R. Weiner, PA-C, is a dedicated physician assistant committed to delivering top-notch, empathetic healthcare to her patients. With a wealth of experience serving diverse communities, Melanie currently practices at Greenwich Primary Care in Stamford, Connecticut. 

After earning her bachelor’s degree in allied health from the University of Connecticut in Storrs, Melanie pursued her passion for healthcare by obtaining her Master of Physician Assistant Studies (MPAS) from Pace University – Lenox Hill Hospital in New York, NY. During her tenure at Pace University, Melanie distinguished herself as a PA didactic student tutor and was honored with induction into the prestigious Pi Alpha – National Honor Society.

Hitesh Patel, MD is a board-certified physician specializing in internal medicine and obesity medicine. He provides expert, compassionate primary care at Premier Healthcare Associates, PC, conveniently located in a state-of-the-art facility near Ocean University Medical Center (OUMC), part of Hackensack Meridian Health, in Brick, New Jersey. 

Dr. Patel offers a wide range of services, including the diagnosis and management of chronic conditions such as thyroid disorders, heart disease, diabetes, COPD, and mental health conditions. His practice also provides preventive care, comprehensive physical exams, medical weight loss programs, and aesthetic treatments like TempSure® Envi for skin tightening. Jessica Taff, MD, is a triple board-certified Hematologist, Medical Oncologist, and Internist.  Through her leadership role at Coastal Hematology & Oncology Center, Dr. Taff is honored to serve the communities of Toms River and Forked River, New Jersey, where she was born and raised. 

Education has always played a central role in Dr. Taff’s life. She earned her undergraduate and medical degrees at Georgetown University in Washington, DC. Dr. Taff went on to complete her residency at New York University Langone Medical Center in New York City, where she was selected to serve as Chief Resident in Internal Medicine. She also completed a Fellowship in Hematology and Oncology at NYU Langone Medical Center, and was again selected to serve as Chief Fellow for Hematology/Oncology. 

While there, Dr. Taff was named Fellow of the Year, and selected to participate in the highly competitive research Conference  ASCO/AACR Workshop in Methods in Clinical Cancer Research in Vail, Colorado, and worked the Bone Marrow Transplant Unit at Memorial Sloan Kettering Cancer Center in New York, NY. Through her extensive research experience helping to develop new and improved cancer treatments, she trained with many of the nation’s top Hematology and Oncology physicians.

Kunal Patel, MD, FACC is a cardiologist and varicose vein specialist at NJ Cardiovascular Institute in Elizabeth and Secaucus, New Jersey. Dr. Patel is known for his friendly demeanor and personalized care, treating each patient like a member of his own family. 

Dr. Patel completed his internal medicine training at Seton Hall University. He then went on to participate in a noninvasive fellowship in cardiovascular disease research at Trinitas Regional Medical Center in Elizabeth, New Jersey, also through Seton Hall University of Health and Medical Sciences. To round out his training, Dr. Patel completed a fellowship in cardiovascular disease at New York Medical College.

Dedicated to his profession, Dr. Patel is board certified in five medical specialties, including cardiovascular disease, internal medicine, echocardiography, nuclear cardiology, and vascular interpretation. Anastasios Manessis, MD, FACE, ECNU, ABOM, is a double-board certified physician at  Endocrine Associates of West Village PC in Murray Hill, Manhattan, and Long Island City, New York, specializing in endocrinology, diabetes, and metabolism and obesity medicine. Dr. Manessis has an endocrine certification in neck ultrasounds and has extensive experience in diagnosing, treating, and managing endocrine disorders, including diabetes, thyroid disorders, hormonal dysfunction, and obesity.  

Dr. Manessis pursued his undergraduate education at Manhattan College, graduating magna cum laude with distinction in biochemistry. He earned his medical degree from the State University of New York at Buffalo. Dr. Manessis completed a combined residency in internal medicine and pediatrics at New York Medical College at Saint Vincent’s Hospital in New York. Following a year as chief medical resident, he completed a two-year clinical fellowship in endocrinology, diabetes, and metabolism, where his research interest, among others, involved the use of insulin pumps in patients with Type II diabetes. 

In addition to his clinical practice, Dr. Manessis was a faculty member in the Department of Medicine at New York Medical College from 2008-2010. He then joined the faculty of New York University in 2011. He has been involved in many clinical research projects, including long-term studies to evaluate the efficacy and safety of GLP-I agonists and weight loss agents. Dr. Manessis’ primary clinical interest includes the treatment of diabetic patients with insulin pumps and how it has benefited the well-being of patients long-term.
