Dr. Gregory R. Sonn graduated from Kansas City University of Medicine and Sciences in 2002 and earned his Bachelor of Arts degree in Biology from Florida Atlantic University.

Dr. Gregory Sonn grew up in Fort Myers and after graduating from medical school, he returned to the Fort Lauderdale area to practice. During this time, Dr. Sonn began treating a great deal of geriatric patients and found that they were largely underrepresented and in need of a dedicated medical provider. After receiving a secondary certification in palliative and hospice care, he relocated to Fort Myers to begin work with Hope Healthcare Services in 2008.

With experience serving as Medical Director for Hope Hospice, Chief Medical Officer at Hope Health Care and a position as Associate Medical Director Dade/Monroe counties with Vitas Innovative Health Systems, it is clear that Dr. Gregory Sonn is passionate about patient care. Dr. Baily has joined us from Pinnacle Health Cardiovascular Institute located in Harrisburg, Pennsylvania. He is a graduate of Northwestern University Medical School in Chicago, Illinois where he originally met and trained under Dr. Talano.  Dr. Baily completed his Residency and Fellowship, Research Fellowship and Chief Fellowship training at Penn State University Hospital in Hershey, Pennsylvania and his teaching appointments include the Milton S. Hershey Medical Center and Penn State University Hospital.

After completing his training he was appointed Assistant Professor of Medicine in the Division of Cardiology and has published several peer reviewed articles in the field of integrative physiology during his tenure at the Pennsylvania State University Hospital.  He later served as the Chief of Cardiology, Holy Spirit Hospital in Camp Hill, Pennsylvania for more than a decade and was also elected Chairman of the Medical Executive Board for Holy Spirit Hospital.
Dr. Baily is Board Certified in Internal Medicine and Cardiovascular Disease and is a Fellow of the American College of Cardiology, the American Heart Association Council on Clinical Cardiology and a member of the American Society of Echocardiography. 

James J. Talano, MHA, FACHE established SWICFT in 2005 alongside his father, and in 2007 assumed the role of Administrative Director and began building the Group. Jim is also the co-founder and Managing Director of Southwest Florida Research, a research center of excellence for the administration of Phase II-IV clinical trials in pharmaceutical and medical devices. Jim received a Bachelor of Arts in Economics from Northwestern University, a Master of Healthcare Administration degree from the University of North Carolina at Chapel Hill and is a Fellow of the American College of Healthcare Executives.

Jim has had a diverse career that has spanned many facets of the health care field. He has held the positions of Director of Managed Care and IT Integration for Northeast Health Systems, an integrated delivery system located outside of Boston, MA. He was a manager with the Chicago office of First Consulting Group specializing in healthcare information technology and strategy consulting. He held the title of Chief Financial Officer and Vice President of Business Development for Medical Systems Management (MSM), a Boston-area-based medical software company, and then went on the become a Vice President with and later serve on the Board of Directors of Picis after helping orchestrate the merger of Picis and MSM. Jim has since been involved in several healthcare-related entrepreneurial ventures and sold a home health and geriatric care management company in 2010 that he developed and grew through the acquisition and merger of two Naples-based companies.

Jim recently served as President of the Western Florida Chapter of the American College of Healthcare Executives. During his time there, he served as the Chief Cardiology Fellow and completed his Internal Medicine Residency and Fellowship in Cardiovascular Disease. Dr. Azad received his undergraduate degree from the University of Miami and is a graduate of Nova Southeastern University Medical School.

He is board certified in Internal Medicine, Cardiovascular Disease, Adult Echocardiography, and Nuclear Cardiology.  He is also board eligible in Vascular Ultrasound and Cardiac CT.  Dr. Azad is a Fellow of the American College of Cardiology and is a member of the American Society of Echocardiography, and the American Society of Nuclear Cardiology.
