Dr. Farha Zibak received her Doctor of Dental Surgery from NYU College of Dentistry, after which she completed her General Dentistry residency at Kingsbrook Jewish Medical Center.

Sliding Fee Scale

LaSante Health Center provides quality medical and dental services to everyone, regardless of a patient’s ability to pay. We accept Medicare, Medicaid and various commercial insurance plans. Insured and uninsured patients may be eligible for discounted rates on a sliding fee scale, based on annual income and family size. Staff are available to assist patients in determining eligibility for participating in the sliding fee scale discount program. Most importantly, no one is denied care due to lack of insurance or inability to pay for services.

Dr. Jared Tuccillo, DMD is a highly compassionate and skilled dentist who provides comprehensive dental care to all of his patients. Dr. Tuccillo grew up in Newtown, Pennsylvania and went on to earn his bachelor’s degree in Kinesiology from Temple University. He then went on to complete a one-year post-baccalaureate program at Temple University’s Kornberg School of Dentistry, and then received his DMD degree from Tufts University School of Dental Medicine in Boston, Massachusetts in 2023. Dr. Tuccillo enjoys all aspects of dentistry and has a particular interest in dental implants as well as restorative dentistry.
