Evelyn Hu is a Family Nurse Practitioner in Colorado who focuses on Family Practice and Primary Care. She founded MedPhone Rx in a mission to provide services to remote patients all over the State of Colorado.
She and the providers of MedPhone Rx can perform face to face video consultations when your doctor is not available, see you at designated local clinics when personal contact is necessary, and even do home visits when possible!

Dr. Hoffenberg is from Rio de Janeiro, Brazil, and speaks fluently Portuguese, Spanish and English. She graduated from Medical School at the Universidade Estadual do Rio de Janeiro, and had clinical training in Pediatrics, General Clinical Preventive Medicine and Addiction Medicine, and completed a Master in Science of Public Health, all at the University of Colorado.
Dr. Ana is clinical faculty at the University of Colorado School of Medicine, where she has acquired, over decades of practice, along with years of work in the private sector, extensive experience treating addicted patients with comorbid psychiatric conditions.
She is excited to now open Your Wellness Journey Center, extending her tender loving care to her new patients here.
Dr. Sarah Mack is a Colorado Native who completed medical school at Rocky Vista University. She received her undergraduate degree in biology with a minor in chemistry at University of Northern Colorado and a masters in Biomedical Sciences from Colorado State University. She then completed residency with Durant Family Medicine in Oklahoma learning to care for a rural population and help an underserved population manage their health. Dr. Mack enjoys caring for whole families and providing thorough care through education and discussion of options.
Dr. Mack enjoys spending time with her husband and family when she’s not practicing medicine. She enjoys being in the mountains as much as possible with hiking, camping, and off-roading with her husband. She enjoys cooking and baking for her loved ones and learning new techniques when she can.
