This company was founded in honor of my late grandmother, Mattie Jackson. I am fortunate to witness as well as experienced my family care so greatly for my grandmother as she aged. She is the driving force behind my passion to care for the elderly. Being a first generation medical doctor, I am pleased to be able to care for ALL ages as a Family Medicine Physician. While specializing in Geriatrics, I find joy in facilitating the day to day issues facing our aging population as well as their families. As Meharry Medical college graduate, I strive to provide the underserved quality care throughout my career. Fortunately, I completed my training in my hometown, Chicago,my residency taught me the value of bringing home care back en vogue for the Geriatric population.

Milton Chávez, MD, is a board-certified family medicine physician at his practice, Dr. Milton Chávez MD, SC in Wicker Park, Chicago. Dr. Chávez takes a proactive approach to medical care and views his role as a patient advocate, exploring all avenues to help them prevent, improve, or maintain health and wellness. 

After completing his Bachelor of Arts in Modern Languages at Wofford College in Spartanburg, North Carolina, Dr. Chávez went on to earn his medical degree at Meharry Medical College in Nashville, Tennessee. He then completed his surgical residency and a pediatric surgical fellowship at the University of California at Los Angeles, before moving to the midwest for his family medicine residency training at Rush University in Chicago. He also completed a mini epilepsy fellowship at Bowman Grey School of Medicine in Winston-Salem, North Carolina. 

Dedicated to his profession, Dr. Chávez is board certified in family medicine and is a Fellow of the American Academy of Family Physicians. He’s also an active member of the Society of Teachers of Family Medicine, the American Academy of Family Physicians, the American Society of Colposcopy and Cervical Pathology, and the American College of Physician Executives. 

In addition to providing compassionate care to all of his patients, Dr. Chávez’s special interests include in-office surgery and procedures, including the diagnosis and management of skin and musculoskeletal disorders.

Dr. Chávez has many interests, but he’s passionate about health, food, travel, and cycling. 

Doctor Cvengros is a Family Medicine physician with a Certificate of Added Qualifications in Sports Medicine. She has special interest in a healthy diet, exercise, and lifestyle to improve and enhance health. She has attended many nutritional conferences focusing on health and nutrition including the Physicians Committee For Responsible Medicine (PCRM) and the Plantrician Project. She shares a private practice with Doctor Gilberto Munoz at Chicago Med Group/ Munoz MD & Cvengros MD, SC. She also precepts the Family Medicine residents at the Access Community Health Network outpatient clinic at Mount Sinai Hospital. Brad Torphy, M.D. is actively involved in research in the field of headache medicine, and he has presented this research at numerous national and international headache conferences. He has co-authored a book on the treatment of headache, and he continues to participate in research into both the factors that may worsen headache as well as novel treatments for various headache conditions. He holds a Certificate of Added Qualification in Headache Medicine from the National Headache Foundation. He spent six years as a Staff Physician at the Diamond Headache Clinic. He was named Physician of the Year at MacNeal Hospital, and he served as Chief Resident at University of Illinois - affiliated St. Joseph Hospital, where he completed his residency training. Dr. Torphy graduated from Xavier University School of Medicine, where he was awarded the President's Scholarship. He received a double-major baccalaureate of arts in Biology and History from Indiana University.

He graduated from Rush Medical School and received his residency training at the Northwestern University-St. Joseph Hospital Family Medicine Program where he also served as chief resident. In addition, he completed an externship in Family Therapy at the Family Institute of Chicago, a Northwestern University affiliate. In addition to 20 plus years as a faculty member of one of Chicago’s leading Family Medicine residency programs, he has worked as a private physician in an ethnically diverse neighborhood of the city. He has extensive experience in hospitalist medicine, and previously served as the Medical Director of an alternative health center focused on the care of AIDS patients. His areas of special interest and expertise include academic evidence based medicine, geriatrics, complex medical cases as well as general family and preventive medicine.

Dr. Jim Wilson is a graduate of the University of Illinois, College of Medicine. He is double board-certified in both Anesthesiology and Pain Medicine. He was trained at both University of Illinois and Michael Reese Hospital. Dr. Wilson has been treating pain for over 30 years. His main interest is in regenerative medicine and will be publishing a safety study using exosomes in lumbar facet arthritis soon.

William C. Campbell, DO, is a board-certified physician with over 15 years of professional experience. He specializes in both family medicine and pain medicine at Health Solutions, with locations in Munster, Indiana, and Tinley Park, Rockford, and Olympia Fields, Illinois. 

Dr. Campbell received his medical degree from Midwestern University Chicago College of Osteopathic Medicine in Downers Grove, Illinois. He went on to complete a residency at Northwestern Memorial Hospital in Chicago.

Throughout his career, Dr. Campbell has received immense training in osteopathic manipulation techniques. He has years of experience treating individuals with various musculoskeletal system problems and dedicates himself to providing long-term, positive results for all of his patients. 

Dr. Campbell is currently the chair of the Family Medicine Department at St. James Hospital in Olympia Fields and has an affiliation with numerous other Franciscan Health Centers. Roy J. Bardwell, DO, is a board-certified family medicine and pain medicine physician with over 10 years of professional experience.
