Welcome to Crescent Beach Care! As a nurse practitioner, I strive to provide holistic care, focusing on health maintenance, health promotion, and disease management. I also value quality, consistent care to everyone. I moved to St. Augustine in 2018 after living in Gainesville for many years. My family and I jumped at the opportunity to join this beautiful beach community where we enjoy everything outdoors, including kayaking, biking and camping. I'm a board-certified Family Nurse Practitioner specializing in holistic health care, working closely with patients to achieve their goals and manage chronic conditions through individualized diet, exercise and medical management. A tried-and-true Floridian, Sara was born and raised in Miami, received her BSN from Florida State University in 2006 and MSN from University of Florida in 2012. St Augustine became her forever home in 2007 and life is full of activity with her amazing husband and four busy kids. When not caring for patients, she is likely to be found on the soccer turf or baseball pitch but tries to squeeze in a beach day or camping trip whenever the opportunity arises!

Originally from Richmond, VA, I did some time in the Army and ended up in Texas, where I met my wife. Our young family decided to move back to Florida get back to the beach and culture. I enjoy procedures, acute and chronic care, and working together with patients to achieve their goals. I enjoy working with patients to prevent and correct any health concerns they may have. I enjoy any hiking and camping, tinkering in the garage, and going to the beach.

Yana Gofman, D.O. is a board-certified physician with extensive experience in primary care, geriatric medicine, and preventative healthcare. She graduated with honors from Nova Southeastern University College of Osteopathic Medicine, where she ranked in the top 10% of her class and was the recipient of the prestigious A. Alvin Greber Cardiology Award. 

Dr. Gofman earned her Bachelor of Science in Biology with a minor in Psychology from Nova Southeastern University in 2007, where she received the Outstanding Student Award. She began her academic journey at Ferris State University, demonstrating early on a strong commitment to excellence in her studies.

Her medical training includes completing an internship and residency in osteopathic internal medicine at Mount Sinai Medical Center in Miami Beach, FL, from 2011 to 2014. During this time, she gained a robust foundation in managing complex medical conditions across diverse patient populations.

Dr. Gofman has over a decade of professional experience as a primary care physician. Her clinical focus is on treating adult and geriatric patients, emphasizing personalized care and preventative medicine to enhance patient outcomes. Dr. Gofman is deeply committed to fostering long-term patient relationships and empowering individuals to achieve their best health through proactive, comprehensive care.

Dr. Gofman is fluent in both English and Russian, which enables her to effectively communicate with a diverse range of patients. Nelson Calvo, APRN is originally from Havana City, Cuba. Growing up he always had a passion for medicine and in 1990 started medical School and graduated in 1996 as a general practitioner in a well-recognized institution, Hospital Calixto Garcia. Nelson then completed his residency program as a Family Physician, ending as a specialist in Urology in 2002. Shortly after, he was sent by the Cuban government to Venezuela as a primary care provider to serve as a doctor in underserved communities.

Working without resources in a community, living below the poverty standard, isolated from health facilities, without economic resources, and with an extremely low level of education, created a new challenge for Mr. Clavo. He then set a goal to reduce comorbidities and mortality associated with parasitism, and chronic/acute disease.

In 2007 he moved to America in hopes of better possibilities for his family and himself. In 2018, after many years of hard work and dedication, Nelson graduated as an Advanced Nurse Practitioner and returned to the Primary Care setting again. Now with more experience, he utilized the tools he learned in medical school, residency, and life to improve and promote health in my community. As a practitioner, his goal is to continue using his extensive experience when treating patients and learning in this fast-evolving world of medicine.

Nelson Calvo, APRN is proud to be happily married with two sons, one of which has already graduated as a Medical Doctor and the other on his way to becoming a physician as well. In his free time, he likes practicing sports, dancing, and listening to music.

Rafael Castells Ferras is a trusted and accountable bilingual (English and Spanish) Board-
Certified Advanced Practice Registered Nurse with over 12 years of experience in the medical field. He is trained to work in high-stress environments, demonstrating excellent
interpersonal communication skills and a keen sense of purpose. Rafael excels at
collaborating with healthcare teams to provide superior medical care, with exceptional

Mr. Castell's medical education in Cuba includes a specialization in Ophthalmology and Visual/Neuro Electrophysiology, with several years of community service. He holds a Master’s in Nursing from Miami Regional University and has trained to work in acute care settings at the University of Miami Emergency Room and Bascom Palmer ER. Since the global health crisis, he has been actively practicing and providing patient care in these critical environments.

Robin Saltz, DO, has been a board-certified family medicine physician in South Florida for 30 years. As a doctor of osteopathic medicine (DO) at AssociatesMD, Dr. Saltz provides exemplary family medicine care for her patients in the Dania Beach, Florida, office. 

Dr. Saltz earned her undergraduate degree at Jacksonville University in Jacksonville, Florida. She went on to the Des Moines University College of Osteopathic Medicine and Surgery, now part of the University of Osteopathic Medicine and Health Sciences, in Des Moines, Iowa, where she earned her DO. 

Dr. Saltz has been a member of the American Osteopathic Association (AOA) and the Florida Osteopathic Board of Family Physicians since 1998.

Deborah Kuiper-Tomas, PA-C, is a Certified Physician Assistant specializing in Family Medicine, Primary Care, Geriatrics, and Dietetic Technology. Deborah also goes by Debi and sees patients at the multi-specialty medical group’s office in Davie, Florida.

Debi completed a Physician Assistant (PA) degree as well as a degree in Dietetic Technology from Miami Dade College located in Miami, Florida, followed by her Master's Degree in Medical Science from Nova Southeastern University (NSU) in Davie.

She has taken multiple courses in plant-based nutrition, including the T. Colin Campbell Plant-Based Nutrition Course at Cornell University certificate program.

Kirenia Santiuste, DNP-PhD-C, is a board-certified family nurse practitioner serving patients at AssociatesMD in Davie, Florida. She holds an Autonomous Practice Endorsement in Florida due to her vast knowledge and expertise.

Kirenia was born and raised in Cuba, where she decided at the age of 5 to dedicate her life to the art of medicine and her patients (who were dolls at the time). Her parents were the engine that would push her to pursue her dreams. Kirenia attended medical school in her native city, bringing several years of competence, experience, evidence-based practice (EBP), and endless commitment to AssociatesMD and the community.

Kirenia has been practicing as a primary care provider since 2016 and holds a Master of Science in nursing and a Bachelor of Science in nursing from Nova Southeastern University in Fort Lauderdale, Florida, where she received The Dean’s Award for Academic Excellence due to her stellar performance as well as the Chancellor’s List Award for five consecutive years. 

Kirenia also practices as a women’s nurse practitioner, certified colposcopist, and mentor, which was granted due to more than 4,000 direct clinical hours in an OB/GYN private practice. Kirenia completed her Doctorate in Nursing Practice core components and combined it with a PhD-candidature at Walden University in Minneapolis, Minnesota. 

Kirenia has been extensively recognized by her colleagues, nursing organizations, and patients for her professionalism, charisma, leadership, patient care, excellence, and commitment to clinical practice, including the NCQA for preventing diabetes and data measurement.

Luce Solari-Suprena, APRN, has been a board-certified family nurse practitioner from the American Nurses Credentialing Center (ANCC) since 2013. She provides compassionate care to patients at AssociatesMD in Margate, Florida. 

Luce received her bachelor’s degree in nursing from Florida Atlantic University in Boca Raton and a master's degree in nursing from Florida International University in Miami, graduating with honors. 

Luce began her nursing career in 2007. As a nurse practitioner, she has worked in various inpatient and outpatient settings, as well as emergency medicine, internal medicine, family medicine, transplant ICU, telemedicine, HIV management, home health, long-term care, mental health, med-surg, and urgent care.

Luce is honored to be a part of AssociatesMD, where her main focus is to provide comprehensive care for every patient in a timely manner while focusing on disease prevention, management, and health promotion. She is passionate about teaching her patients ways to improve their health. Strong communication skills and a kind soul have been Luce’s assets in her long career dedicated to service.

She is fluent in Creole, French, and English. In her spare time, Luce enjoys spending time with her family, reading, doing beach activities, dancing, and traveling.
