Jennifer Hall DiFalco, MD, is a family physician with more than two decades of experience providing high-quality care to patients of all ages. She offers comprehensive family health care at Bethel Family Medicine in Brockton, Massachusetts.

Dr. DiFalco joined the team at Bethel Family Medicine in 1994. In 1998, she embarked on her service as a missionary, which eventually led her to the Ivory Coast in West Africa, where she spent 12 years as the medical director of the Baptist Mission Hospital in Ferkessédougou, Côte d’Ivoire. During this time, Dr. DiFalco became fluent in French and gained a wealth of insight into cross-cultural medicine.

Since rejoining Bethel Family Medicine in 2012, Dr. DiFalco has become the resource adviser for electronic medical records in addition to her clinical duties. She handles medical exams for commercial drivers and is certified by the Department of Transportation to carry out examinations.

A native of Massachusetts, Dr. DiFalco grew up in East Bridgewater. She graduated magna cum laude with a degree in biology and chemistry from Gettysburg College in Pennsylvania. She earned her medical degree from the University of Massachusetts Medical School in Worcester and completed her residency at Williamsport Regional Medical Center in Pennsylvania.

In her free time, Dr. DiFalco is active in her church’s outreach ministry, Operation Kindness, and enjoys spending time with her husband in their new home.

I started Integrative Health & Wellness Clinic with the mission to provide high quality, comprehensive & holistic care, so that clients may find and live with their best health. Education background: In 1999, I graduated from the New Hampshire Technical Institute (NHTI) with an Associate’s Degree in Nursing. While at the NHTI, I was awarded by the nursing facility "The Goldie Crocker Award" for outstanding in the Art and Science of Nursing. I continued my nursing studies and in 2011, I completed my Masters in Nursing with honors and achieving my board certification through the American Nurses Credentialing Center, as a Family Nurse Practitioner. In addition to my university studies I have achieved certification as a level II Reiki practitioner, Certification in Integrative Nutrition through the Institute of Integrative Nutrition and completed the course work on Functional Medicine through the Institute of Functional Medicine.

Dr. John Andrea is a board-certified physician in both Family Medicine and Sports Medicine. He earned his Bachelor of Science in Psychology from the University of Massachusetts Amherst and his Doctor of Osteopathic Medicine from the University of New England in Biddeford, Maine. He completed his medical training with a Family Medicine residency at Fairfax Family Practice in Virginia, followed by a Sports Medicine Fellowship at INOVA Fairfax, graduating in 2021.

As a primary care physician, Dr. Andrea cares for patients of all ages and has special interests in non-operative orthopedics, sports medicine, musculoskeletal ultrasound, procedures, injections, and Osteopathic Manipulative Medicine (OMM). He is passionate about prevention, staying active at every stage of life, and building long-term relationships with his patients. Dr. Andrea stays current through ongoing education and sports medicine training and also volunteers with the U.S. Olympics. Dr. Rajesh Shoor is a board-certified Internal Medicine physician with extensive experience providing compassionate, high-quality care. He earned his Bachelor of Arts with High Distinction from the University of Virginia in Charlottesville and went on to complete his Doctor of Osteopathic Medicine at the University of Medicine and Dentistry of New Jersey in Stratford, NJ. Dr. Shoor completed his residency training in Internal Medicine in 2005 at the University of Medicine and Dentistry of New Jersey in Newark.

Following residency, Dr. Shoor served as a physician in the U.S. Army, including a 15-month deployment in Iraq, where he cared for soldiers in demanding environments. In 2008, he relocated to Southern New Hampshire, where he has since been dedicated to serving the local community through roles at Parkland Medical Center in Derry and Catholic Medical Center in Manchester.

Dr. Shoor is excited to welcome new patients to Southern New Hampshire Internal Medicine. He is committed to building lasting relationships with patients and delivering personalized care focused on wellness and prevention.

Dr. Yuliya  Mandel is a Board Certified Family Physician who joined SNHIMA in 2024 after relocating to The Granite State.  She brings  over 15 years of experience in the primary care field , with a strong commitment to delivering high-quality, patient-centered healthcare.

Dr. Mandel earned her medical degree from the Crimean Medical Institute and completed her postgraduate residency training at St. John’s Hospital and Medical Center in Detroit, MI.  Upon completion of her residency, she founded and managed a successful primary care practice in North Andover, MA, where she cared for hundreds of patients over the span of more than a decade.

She has chosen to continue her career at SNHIMA, as our philosophy of providing personalized, compassionate care aligns with her own.
