Alexander Dydyk, DO, is a pain management specialist. He serves patients at HealthyU Clinics at our Mesa, Avondale, Glendale, Gilbert, Surprise and Phoenix, Arizona, clinics.  Dr. Dydyk joined HealthyU Clinics in 2022 as their director of pain medicine, weight loss, and wellness. 

Dr. Dydyk completed his bachelor's degree in biology from Arizona State University in Tempe. He then completed his master's degree in biomedical sciences and Doctor of Osteopathic Medicine from Midwestern University in Glendale, Arizona.

After graduating from medical school, he completed residency training at the Abrazo Family Medicine Residency Program in Phoenix, Arizona, where he served as chief resident.

After residency, Dr. Dydyk finished two one-year ACGME-accredited fellowships. He completed the Long Beach Memorial Primary Care Sports Medicine Fellowship Program in Long Beach, California, followed by the Allegheny Health Network Pain Medicine Fellowship in Pittsburgh, Pennsylvania. In 2023, he obtained his obesity medicine certification from the American Board of Obesity Medicine.

Dr. Dydyk’s free time is primarily spent with his wife, three young children, extended family, and friends. Nothing is better for him than making breakfast with kids on the weekends or going out with his wife. He loves cooking, going to the gym and lifting weights, watching movies, and playing professional basketball.

As a lifelong Phoenix Suns fan, he hopes one day the Suns will hang a championship banner in their rafters.

A southern Californian native, Dr. Lai completed his medical training at Tufts University where he subsequently served as a clinical instructor. He is board certified in Physical Medicine and Rehabilitation. He also holds board certifications in Pain Medicine and Independent Medical Examiners. 

Dr. Lai’s primary goal is to achieve maximal restoration of physical, psychological, social and vocational function through comprehensive rehabilitation. Dr. Lai’s professional interests include interventional pain management, neuromusculoskeletal disorders, electrodiagnostics, industrial and occupational rehabilitation. His approach to caring is based on improvement in quality of life through improvement in function.

Dr. Shinto Koshy completed his Physical Medicine and Rehabilitation residency training at the State University of New York Downstate and is board certified. He completed his Interventional Spine Fellowship at Centers for Rehabilitation and Pain Medicine.

Dr. Koshy is an active member with the American Academy of Physical Medicine and Rehabilitation, Association of Academic Physiatrists, American College of Healthcare Executives, and the American Society of Interventional Pain Physicians. Dr. Radahd joined the Pain Medicine Group after completing a Physical Medicine and Rehabilitation residency at the University of South Florida and a fellowship in Interventional Pain Management at Virginia Commonwealth University. She is Board Certified in both Physical Medicine and Rehabilitation and Pain Medicine. She specializes in treatment of neck and low back pain through the use of interventional procedures and spinal cord stimulation. She also treats a variety of injuries, including back pain and osteoarthritis, with regenerative medicine. She is excited to offer her services to the community.

Dr. Jonathan Chin, the founder of Excelsior Pain Management, grew up in Austin, TX, and even though his medical expertise has taken him around the country, he has always identified as a proud Austinite. Dr. Chin graduated with high honors from the University of Texas at Austin and later graduated from medical school at the University of Texas Southwestern Medical Center at Dallas.

Following medical school, Dr. Chin moved to Houston and completed his residency in Anesthesiology at Baylor College of Medicine, which is consistently ranked as one of the best Anesthesiology programs in the country.  Desiring further subspecialty training, Dr. Chin was selected among thousands of applicants to complete a Pain Fellowship at the prestigious Harvard Medical School’s Brigham and Women’s Hospital, a program that is ranked the #1 Pain Fellowship program in the country by both the American Pain Society and the American Academy of Pain Medicine.

While at Harvard, Dr. Chin cultivated a passion for treating chronic pain patients – training under some of the pre-eminent pioneers and thought-leaders in the field of Pain Medicine.  During this time, driven by a desire to innovate and improve pain outcomes for patients, Dr. Chin also worked on an FDA-approved multi-center clinical research trial to develop and test a novel, non-opioid pain medication.

After graduation, Dr. Chin returned to the Austin area of Texas to serve as an Assistant Professor in the Department of Anesthesiology and Pain Medicine at Texas A&M medical school through a local hospital, where he spear-headed studies to improve pain outcomes in patients with Painful Diabetic Peripheral Neuropathy, and to improve pain outcomes as well as sensory and motor deficits in patients with Spinal Cord Injuries.

Due to his passion for excellence in delivering pain treatments, Dr. Chin founded Excelsior Pain Management.  He knew that starting his own clinic would be the only way to ensure his patients received the highest level of personalized care.  It is his goal that the training he received from the top institutions in the world, coupled with his passion and care, will help him treat his patients’ wide spectrum of pain conditions – no matter how chronic or debilitating they may be.  His mission is to ensure that every patient is treated with kindness, dignity, and the most cutting-edge therapies for chronic pain.

Dr. Chin’s current clinical interests include the use of Neuromodulation, Spinal Cord Stimulation, and Dorsal Root Ganglion Stimulation to treat various otherwise treatment-refractory pain conditions.  Dr. Chin also has a provisionally patented medical device, and with the support of Harvard and MIT faculty, plans to finish development of the device to change the field of pain management for the better.

Outside his clinic, you can find Dr. Chin with his wife and family enjoying all Austin has to offer.  As a prior NCAA athlete and member of the Texas Crew at the University of Texas (Go Longhorns!), he still enjoys rowing and sports.  He loves serving his church community and is an active church member in the Diocese of Austin.

Dr. Gleis is board certified by the American Board of Anesthesiology and is licensed in Florida and Pennsylvania. He is involved with multiple organizations including the American Society of Anesthesiologists, the American Society of Interventional Pain Physicians and the Regional Anesthesia and Pain Medicine Research Society. Dr. Gleis employs a multimodal approach to pain management, treating his patients’ symptoms through interventional pain procedures and non-habit-forming medications if required, without the use of narcotics. Dr. Gleis also has extensive experience in addiction medicine having previously served as the medical director for a prominent drug and alcohol treatment center.

Justin M Raye, DO, is a board-certified physical medicine and rehabilitation (PM&R) physician and interventional pain management specialist at 360 Orthopedics in Sarasota, Venice, and Lakewood Ranch, Florida.

Dr. Raye earned his Bachelor of Science in applied physiology and kinesiology (Summa Cum Laude) at the University of Florida. He then completed his Doctor of Osteopathic Medicine degree at DeBusk College of Osteopathic Medicine in Tennessee, where he graduated in the top 10% of his class. He completed his internship at Advocate Illinois Masonic Medical Center, before completing his PM&R residency at the University of Minnesota, where he was also named Chief Resident. Dr. Raye’s research appears in several medical journals and his clinical trials have proven valuable in stem cell repair and spinal stenosis treatment. Throughout his medical training, Dr. Raye has provided medical coverage at the USA Cup Soccer Tournament, Twin Cities Marathon, Susan Komen 3-Day Event, and the NCAA Division II men’s soccer team at Lincoln Memorial University in Tennessee.

Dr. Raye is a native of Bradenton, returning to his hometown to give back to our community. During his free time, he enjoys soccer, football, basketball, biking, fishing, boating, the beach, traveling with family and friends, and, of course, Florida Gator athletics.

Elvis Guzman, MD, is an interventional pain management physician at Florida Pain Management Institute, serving patients in Delray Beach and Boynton Beach, Florida. He brings twenty years of experience and knowledge to Beverly Hills Migraine and Pain Management Institute, serving patients in Beverly Hills and Encino, California. 

Dr. Taimoorazy is one of only seven interventional pain management anesthesiologists in the nation and the only physician in California who is also sub-specialized in the field of headache medicine by the United Council for Neurologic Subspecialties. 

He is a fellow of the American Headache Society, an elite member of The Leading Physicians of the World, and was recognized as one of the few top interventional pain management anesthesiologists in the nation.
