Jerleshia Fields, ARNP-BC is a family nurse practitioner with experience in medical-surgical, progressive, and cardiovascular care, as well as in operating room and home health settings. She received his bachelor’s and master’s nursing degrees from the University of Phoenix in Jacksonville, Florida, and her family nurse practitioner post-master certificate from Indiana State University in Terre Haute, Indiana.
Rejuvené Family Medicine was started by Rachel Sirois, APRN, FNP-BC. She has more than 12 years experience in healthcare including almost 3 years as a family nurse practitioner. She is a graduate of Jacksonville University and her first job as a nurse practitioner was in Macclenny at a convenient care clinic. It was during that time that she started noticing how many patients did not have a primary care provider. During this time, Rachel and her family fell in love with the small town feel of Macclenny and decided to purchase land, build a house, and plant some roots. They officially became Baker county residents in January 2022 and Rachel immediately started planning her clinic. She hopes that opening this clinic will help close the gap in the lack of primary care here and provide a safe, friendly, caring, and Christ-like environment for her patients. Whether you have Medicare, BCBS, or no insurance, she hopes to help make the residents of Baker county healthier!
I'm a board-certified Family Nurse Practitioner specializing in holistic health care, working closely with patients to achieve their goals and manage chronic conditions through individualized diet, exercise and medical management. A tried-and-true Floridian, Sara was born and raised in Miami, received her BSN from Florida State University in 2006 and MSN from University of Florida in 2012. St Augustine became her forever home in 2007 and life is full of activity with her amazing husband and four busy kids. When not caring for patients, she is likely to be found on the soccer turf or baseball pitch but tries to squeeze in a beach day or camping trip whenever the opportunity arises!

The results required a tele-health a few days later with ARNP Sara Bruce. She was great and explained some additional steps to follow. I am very happy to have these folks guiding and caring for me. As a nurse practitioner, I strive to provide holistic care, focusing on health maintenance, health promotion, and disease management. I also value quality, consistent care to everyone. I moved to St. Augustine in 2018 after living in Gainesville for many years. My family and I jumped at the opportunity to join this beautiful beach community where we enjoy everything outdoors, including kayaking, biking and camping. I am proud to be a member of the Crescent Beach Care team of providers. I look forward to personalizing care for each of my patients and their families.
