Originally from Richmond, VA, I did some time in the Army and ended up in Texas, where I met my wife. Our young family decided to move back to Florida get back to the beach and culture. I enjoy procedures, acute and chronic care, and working together with patients to achieve their goals. I enjoy working with patients to prevent and correct any health concerns they may have. I enjoy any hiking and camping, tinkering in the garage, and going to the beach.
“Crescent Beach care is a place that you are in good hands. Doctor Jacob Dohogne is a caring, concerned and patient position. You can certainly tell he reviewed your records prior to meeting with you. Always feel such a sense of personalized care when I speak with him. He’s not only concerned about what I am speaking about on the call or the visit, there is also his expertise in any other matters that you wish to discuss. The staff is friendly and try to be as accommodating as possible. Thank you!”
“Dr. Thor is an amazing physician and astute diagnostician. She is very down to earth making you feel comfortable and nonjudgement in discussing health concerns. She is also a great detective and thinks outside the box to find the solution to your health concern. She had been my physician for over a decade and I would not see any other doctor. The office is kind and responsive to appointment requests and other questions. Thank you for your service to our commumity!”
Welcome to Crescent Beach Care!
As a nurse practitioner, I strive to provide holistic care, focusing on health maintenance, health promotion, and disease management. I also value quality, consistent care to everyone. I moved to St. Augustine in 2018 after living in Gainesville for many years. My family and I jumped at the opportunity to join this beautiful beach community where we enjoy everything outdoors, including kayaking, biking and camping. I am proud to be a member of the Crescent Beach Care team of providers. I look forward to personalizing care for each of my patients and their families.

I'm a board-certified Family Nurse Practitioner specializing in holistic health care, working closely with patients to achieve their goals and manage chronic conditions through individualized diet, exercise and medical management. A tried-and-true Floridian, Sara was born and raised in Miami, received her BSN from Florida State University in 2006 and MSN from University of Florida in 2012. St Augustine became her forever home in 2007 and life is full of activity with her amazing husband and four busy kids. When not caring for patients, she is likely to be found on the soccer turf or baseball pitch but tries to squeeze in a beach day or camping trip whenever the opportunity arises!
“Very kind, professional, concerned. Knowledgeable and shares information re: disease process and treatment. Allows time for questions, doesn't rush!! Highly recommend!!”
