Dr. Meshi has a special interest in women’s health, including reproductive psychiatric conditions. Hazel Parrish, LCADC, ICADC, BIP, is a licensed alcohol and drug counselor, author, and the founder and owner of Bridge the Gap Addiction and Mental Health Services, PLLC, in Louisville, Kentucky. She has over 30 years of experience in the field of mental health and looks forward to offering compassionate care to those in the greater Louisville metro area. 

Professionally, Hazel wears many hats; she is a licensed clinical alcohol and drug counselor, a certified marriage and family therapist, peer support specialist, domestic violence batterer intervention provider (BIP), an ordained minister, and a moral reconation therapist (MRT). She is also a member of the International Certificiation and Reciprocity Consortorium (IC&RC). 

Hazel graciously offers mobile visits and telehealth services to her clients while also providing a safe space for them to visit in her office. 

Hazel started her education in social services at Peralta College in Alameda County, California. After earning her associate degree, she attended California State University, East Bay in Hayward, California, for her bachelor’s degree in sociology and her Master of Science degree in educational psychology. She started Bridge the Gap Addiction and Mental Health Services, PLLC, in 2004. 

Throughout her career, Hazel has established herself as a highly qualified provider of rehabilitative services for substance abuse, domestic violence, and numerous mental health diagnoses. 

She offers compassionate counseling services in group and individual settings and facilitates intensive outpatient treatment programs and residential treatment. She also oversees recovery programs, including Alcoholics Anonymous (AA). 

After detailed assessments, Hazel guides patients through the process of recovery with a holistic and integrative approach. 

In her personal life, Hazel is a mother to two children and a grandmother of eight. She is also active in her church as an assistant minister. She is currently working on expanding her practice to California while also working on new projects here in Louisville. 

I am a traditionally trained Psychiatrist, with double Board Certification status. After completing medical school and a first residency in psychiatry in Egypt, my journey brought me to the State University of New York, Downstate Medical Center, in Brooklyn, where I completed my four-year psychiatry residency training program. I subsequently joined the Yale University School of Medicine Law and Psychiatry program where I completed my fellowship in Forensic Psychiatry. My eclectic professional experience included working as a volunteer psychiatrist in Egypt with African and Middle Eastern refugees, survivors of torture and sexual traumas, working with 9/11 survivors and first responders during my years in New York, as well as LGBTQ+ refugees and asylum seekers in both the Middle East and the United States. After graduating from Yale, I was appointed as an Assistant Clinical Professor of Psychiatry at the Yale School of Medicine.

Michael Lara, MD, is a skilled, board-certified psychiatrist at his private practice in Belmont, California. He specializes in psychopharmacology and treating mood disorders, anxiety disorders, and attention-deficit/hyperactivity disorder (ADHD) in adults between the ages of 21 and 65.

Dr. Lara earned his bachelor’s degree in English literature with a minor in biological sciences at Stanford University in Stanford, California. He completed his master’s degree in English and American literature and language at Harvard University in Cambridge, Massachusetts. 

Dr. Lara earned his medical degree from the University of California in Los Angeles, and he’s board-certified in psychiatry by the American Board of Psychiatry and Neurology. He went on to complete an internship and residency in adult psychiatry at the Department of Psychiatry and Behavioral Sciences at Stanford Health Care in Stanford, California. 

Dr. Lara served as a chief psychiatrist and medical director at Centro de Bienestar in San Jose, California, a community-based mental health clinic. While there, he managed five psychiatrists and developed in-house psychopharmacology training programs. 

He has also worked in private practice in Belmont, California, where he specialized in anxiety disorders, mood disorders, and ADHD. In addition, Dr. Lara has held faculty positions at the Institute of Brain Potential in Los Altos, California, and Sophia University in Palo Alto, California. 

He uses sophisticated psychopharmacology treatments and is easily accessible to his patients between visits. Dr. Lara also offers telemedicine services for added convenience.

She is skilled at working with sexual assault survivors, trauma, eating disorders, depression, stress and anxiety. As a holistic therapist she views emotional health through the lenses of mind, body and spirit. She also uses expressive art therapy, EFT, EMDR and mindfulness approaches to work on multiple levels of healing.

M Rameen Ghorieshi, MD, MPH, of Palo Alto Mind Body in Palo Alto, California, cares for patients from the paradigm that mental and physical health are so strongly intertwined that one may not fully achieve either without striving for both. He is board-certified in Psychiatry, board-certified in Addiction Medicine, Adjunct Clinical Assistant Professor at the Stanford University School of Medicine, and Medical Staff at Stanford Health Care.

He completed his training at Stanford Hospital and Clinics and attended medical school at the Emory University School of Medicine, where he also received an advanced degree in behavioral sciences and health education from the Rollins School of Public Health. He treats a vast range of psychiatric conditions and helps those who wish to better understand themselves and their relationships. 

His particular areas of interest include anxiety, depression, addiction medicine, whole body health, and coping with chronic or terminal medical illnesses. To best meet your specific needs, Dr. Ghorieshi will formulate a customized treatment plan, which may include psychotherapy, medications, and behavioral techniques.

Concierge Psychiatry

 

Gregory Gale, MD, is a board-certified psychiatrist treating patients at Ocean Psychiatry in Freemont, California. He has over 20 years of experience and expertise in behavioral health, working in various roles in mental health and substance abuse.

Dr. Gale has served in medical leadership roles for direct care community organizations and managed care organizations. For the past five years, he has been the medical director at Acceptance Recovery Center, specializing in co-occurring psychiatric and addiction conditions. At the center, he has oversight of the rehabilitation program. 

Previously, Dr. Gale provided medical leadership as the chief medical officer to various behavioral health provider organizations, including Lifewell Behavioral Wellness and Partners in Recovery. In these roles, he developed integrated behavioral and physical health clinics to treat the whole person, mind, and body.

Concurrently, he serves in a leadership role as a senior medical director in managed care at Molina Complete Care. Previously, Dr. Gale was the medical director at Cenpatico Integrated Care and Humana Behavioral Health. 

Earlier in his career, Dr. Gale was chief of psychiatry for hospital-based care serving the San Francisco Bay area population. There, he transformed the department of psychiatry by shifting the focus to recovery principles and practices from what was a traditional model of care.

Dr. Gale is a graduate of DePaul University. He received his Bachelor of Science in finance with honors. He then completed medical school at Northwestern University in Evanston, Illinois, and his residency at Harvard Medical School in Boston, Massachusetts.

Shareh Ghani, MD, is the founder and medical director of Ocean Psychiatry in Fremont, California. Dr. Ghani is a board-certified psychiatrist and neurologist with over 20 years of experience in the mental health industry. He is licensed to see patients in California and Arizona. 

Dr. Ghani began his career in inpatient psychiatry and worked as a consultant and liaison psychiatrist for many years. Additionally, he has worked as a psychiatrist in traditional ambulatory care settings and urgent psychiatric care environments. 

Dr. Ghani previously served as the chief medical officer for Magellan Health Services for the Maricopa Contract in Phoenix, Arizona, from 2010 through 2014. From 2015 to 2020, Dr. Ghani served as Magellan’s vice president and medical director.

His specialties include depression, anxiety, ADHD, OCD, PTSD, panic disorders, sleep disorders, and eating disorders.

Dr. Ghani currently accepts the following health plans: Anthem Blue Cross, Blue Shield of CA – Magellan, United Healthcare, Sutter Health, Kaiser, Cigna, Aetna, Medicare, Caremore, Alignment, and Tricare.

Michelle Kluczinske, PMHNP-BC, is an experienced psychiatric and mental health nurse practitioner at Ocean Psychiatry in Freemont, California. At the practice, Michelle offers compassionate and effective care to every patient she sees. 

Michelle has experience in psychotropic medication management in outpatient and long-term care settings. She worked with patients 16 years old and over who experienced depression, anxiety, bipolar disorder, PTSD, ADHD, and psychosis. 

Her nursing experience includes working in inpatient psychiatric units, a psychiatric emergency department, psychiatric outpatient clinics, day programs, substance rehabilitation, and interventional psychiatry. Michelle has also been a clinical educator for the psychiatric-mental health rotation at nursing colleges. 

Michelle believes in empowering patients through education and encouraging them to be active and collaborative in their care. She takes the time to really listen to her patients and partners with them to improve their health.

Peter Pham, PMHNP-BC, is a board-certified psychiatric mental health nurse practitioner at Black Onyx, providing complete psychiatric care to people in Orange and South San Francisco, California. 

Peter began his professional pursuits at the Charles R. Drew University of Medicine and Science in Los Angeles, California, earning his graduate degree in psychiatry. He excelled in the program, receiving extensive training in the diagnosis and treatment of various behavioral disorders, including attention-deficit hyperactivity disorder (ADHD), anxiety, and depression.

In 2021, Peter joined the Black Onyx team. He speaks English and Vietnamese fluently and treats people of all ages and backgrounds, but he has a particular interest in working with teens and young adults.

Peter takes a conservative and holistic approach to treatment. Before developing care plans, he considers all aspects of a person’s well-being, including their physical, emotional, and spiritual health. Then, he uses that information to make personalized recommendations.

Peter’s knowledge of various medications and therapies ensures lasting treatment outcomes, allowing patients to live their best lives.

Dr. Anise Tatiana Wheeler, DNP, FNP. ANP, PMHNP. I am a doctorate prepared-nurse practitioner holding dual certifications in psychiatry and family medicine. I hold a master degree in Health Care Management from Johns Hopkins University. I have also received my certification in Transcranial Magnetic Stimulation (TMS) from Duke University School of Medicine. I seek to help my clients achieve their optimal state of wellness--mind, body & spirit, by using individualized evidence-based holistic approaches. I specialize in the management of mental disorders of adolescents ages 14 to 18, adults ages 18 to 65, and elderlies 65+. I combine conventional medication management with psychotherapy, psycho-nutrition (including daily intake of brain booster vitamins. My experience includes treating depression, anxiety, bipolar, spectrum disorders, PTSD, psychosis, ADHD, Addiction, stress, fibromyalgia, pain psychology, and insomnia. I currently collaborate with Dr. Kristel Carrington MD.
