Arise Uwizeyimana

Arise Uwizeyimana is a board-certified Family Nurse Practitioner with over 20 years of healthcare experience, including 15 years in nursing home and rehabilitation care and 6 years as a primary care provider. She is passionate about helping patients restore their health and achieve sustainable weight loss through evidence-based, individualized care. Her clinical focus is cardiometabolic health, including obesity, type 2 diabetes, insulin resistance, hypertension, abnormal cholesterol, and cardiovascular risk prevention. She works with patients to improve metabolic function, reduce long-term disease risk, and build sustainable lifestyle habits. Arise completed her undergraduate studies in Iowa and earned her Master of Science in Nursing from Indiana Wesleyan University. Outside of practice, she enjoys spending time with family, traveling, and hiking. Her mission is to empower patients to take control of their health and achieve lasting wellness.
