Born and raised in Mississippi, Centeria Woolfolk is a board-certified Family Nurse Practitioner and the visionary founder behind Emerald WELL, PLLC — a modern healthcare sanctuary committed to accessible, relationship-based care in Yazoo and surrounding rural communities. With more than a decade of experience in nursing and advanced practice, Centeria has spent her career serving others — from high-acuity hospitals to rural clinics. But it was the gaps in care, the rushed appointments, and the underserved women in her own community that sparked a deeper mission: to create a wellness space where self-care and healthcare intersect. Emerald WELL, PLLC was born from that vision. Through direct primary care, women’s health services, weight management, and affordable wellness offerings, Centeria is redefining what it means to receive whole-person care — no insurance hoops, no judgements, and no short visits!
