I have been to other physical therapy places before this visit . I went in skeptical but left very happy . Chad is very good at what he does . I like the one on one time he gives each patient. You dont feel like a number but a rShow moreeal person . He really cares how you feel . I am half way done with my visits and am amazed at how far I ve come with Chads help. I highly recommend Pro Form physical therapy to anyone.
The clinic is conveniently located. It is modern and well outfitted with all the equipment one would expect to find, and more. Dr. Chad is very knowledgeable and communicates well with my other medical providers. He has been abShow morele to motivate me to push through some painful parts of the recovery process and I am regaining full function sooner than I expected.

Chad is a wonder worker. Professional, caring and knows his PT stuff. My 30-year ankle injury has never felt better! I now have mobility and stability and I wish I had started working with him earlier. Highly recommend Chad Burnham. If you have a pain, you don't need to live with it!
