Dean C. Perfetti, MD, MPH, is a fellowship-trained Orthopaedic Surgeon specializing in spine surgery. Dr. Perfetti sees patients at Somers Orthopaedic Surgery & Sports Medicine Group at their Mount Kisco, Carmel, Newburgh locations in New York, and their Danbury location in Connecticut.
Dr. Perfetti obtained his undergraduate degree at Columbia College (Columbia University), graduating with the highest honors of Summa Cum Laude and Phi Beta Kappa.
He achieved a dual Medical Degree and Master of Public Health Degree from the State University of New York Downstate College of Medicine. He graduated at the top of his class with the highest honors of Alpha Omega Alpha and Beta Ioda Chapter of Delta Omega.
He completed a residency in Orthopaedic Surgery at the Zucker School of Medicine at Hofstra Northwell as chief resident, followed by a spine surgery fellowship at the prestigious Texas Back Institute, which pioneered Total Disc Replacement in the United States.
Dr. Perfetti’s clinical focus is on caring for back and neck injuries, whether from acute traumas such as herniations or fractures, chronic conditions like arthritis and stenosis, or spinal abnormalities.
He is recognized as a 2025 NY Top Doctor by NY Top Docs (USA Top Docs Division). His goal is to promote the health of the vertebral column, spinal cord, and nerve roots to decrease pain and improve function for his patients, ultimately getting them back to their active lifestyles.
Dr. Perfetti is trained in complex cervical, thoracic, and lumbar spine surgeries with a special focus on motion-preserving procedures, like Total Disc Replacement and Minimally Invasive Spine Surgery.
He utilizes the latest robotics and navigation technologies and performs Orthopaedic procedures for extremity fractures.
Dr. Perfetti grew up in Scarsdale, New York, and played soccer for F.C. Westchester, winning three New York State Championships and a National Championship at Disney. He transitioned to football at Columbia, where he played four years as a kicker/punter at the Division 1 level.

Really seems to care
On time with appointment which is unheard of lately. Pleased with how DR explained my MRI results and how it affected my issues. Took time and not rushed through appointment. Listened well and assessed issues with good recommendations.

Dr Carlesi has been my doctor for over a decade. I had been searching for several years to find the right doctor for me. Even though I am an RN and in the medical field for over 30 years, it was not that simple for me. After an injury causing herniated discs, etc.,… which caused unbearable pain almost constantly which in the beginning It felt like it started from the top of my head to my toes. I had previously seen,orthopedic specialists/pain management, neurologist, who were telling me there was nothing they could do for me. For some reason, their first thought was invasive surgery and two of the doctors said that I would not even survive surgery so there was nothing they could do for me I was overwhelmed physically and mentally. How would I be able to live the rest of my life feeling the way I did at that time. Then I was referred to Dr. CARLESI !! He is not only extremely knowledgeable, Think out of the box but he is one of the most compassionate people and Drs I have met. There ARE alternatives other than major surgery that can at least lessen the pain !!! I searched for a couple years to find the right Dr until I got it right. I absolutely recommend Dr Carlesi to anyone who is looking for relief from the constant overwhelming suffering, that people like me with chronic and acute pain deal with.

I’ve seen three doctors from Somers and my daughter had seen and both of us needed surgeries. All doctors, staff we very friendly and helpful. They answered all of our questions and took the time to guide us throughout diagnosis and recovery. I highly recommend using Somers Orthopedics. There are several locations also to accommodate a large area.

Very caring ang concerned about me and what i am going through with my condition
Very difficult surgery and complications. I’m still experiencing the same pain, after 3 weeks. I’m very disappointed I developed a hematoma near my spine. I hope I do not get an infection. Perhaps the drain wasn’t monitored correctly. I know I had serious surgery but very disappointed with the outcome.
