Evette Facey

At Initiare Healthcare Services, led by Evette Facey, NP, based in Jamaica, New York, we provide superior primary care through telehealth for adults 20 and older.

Evette began her educational journey at Medgar Evers College in Brooklyn, where she earned her Associate of Science in Nursing. She went on to earn her Bachelor of Science in Nursing at SUNY Downstate University Center in Brooklyn, New York, and completed her Master of Science in Nursing at Hunter College in New York, New York.

She is a licensed advanced practice nurse and registered nurse in New York, certified by the American Nurses Credentialing Center (AGPCNP-BC) and the American Heart Association (BLS). Evette is also a member of the American Association of Nurse Practitioners (AANP).
