Jasmine Vongphachanh is a Family Nurse Practitioner who believes in building strong relationships with patients through compassionate, personalized, and evidence-based care. She enjoys helping patients better understand their health while creating realistic treatment plans that support long-term wellness and prevention.
Her clinical experience includes internal medicine, acute care, post-operative recovery, and oncology. This diverse background has allowed her to care for patients across a wide range of healthcare settings and has strengthened her skills in chronic disease management, patient education, wound care, and care coordination.
Jasmine earned her Master of Science in Nursing with a Family Nurse Practitioner focus from Texas Woman’s University. During her graduate studies, she was honored for academic excellence through the Phi Kappa Phi Honor Society. She began her nursing journey at the University of Texas at Arlington, completing her Bachelor of Science in Nursing. Jasmine is a board‑certified Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP) and is licensed to practice by the Texas Board of Nursing.
She values creating a welcoming environment where patients feel heard, supported, and involved in their care. Outside of work, Jasmine enjoys traveling, spending time with family and friends, and relaxing with her dog and cat.
