At The Joseph P. Addabbo Family Health Center (JPAFHC), we believe healthcare should be accessible, compassionate, and centered around you. With locations in Red Hook, Brooklyn, and Arverne, Far Rockaway, and Jamaica, Queens, our experienced team provides comprehensive services designed to support the whole person and improve long-term health outcomes.

For nearly 40 years, we have proudly served the communities of Southeast Queens and Red Hook, Brooklyn, providing high-quality, community-based healthcare for generations of families. As a Federally Qualified Health Center (FQHC), we are committed to expanding access to coordinated, patient-centered care regardless of a patient’s ability to pay. 

At JPAFHC, we deliver integrated, whole-person care that combines medical, behavioral, dental, and supportive services under one healthcare network. Our care model focuses on prevention, early intervention, chronic disease management, and wellness while building strong relationships with the communities we serve.

We provide a wide range of primary and preventive care services at JPAFHC, including routine checkups and physicals, as well as specialized care such as chronic disease management. We also provide women's health and family planning services, pediatric care, and dental and oral health services for the whole family. Patients also have access to behavioral health, podiatry, and pharmacy services at JPAFHC, and telehealth appointments are available to help make patient care more accessible.
