For nearly 40 years, we have proudly served the communities of Southeast Queens and Red Hook, Brooklyn, providing high-quality, community-based healthcare for generations of families. As a Federally Qualified Health Center (FQHC), we are committed to expanding access to coordinated, patient-centered care regardless of a patient’s ability to pay.

Patients also have access to behavioral health, podiatry, and pharmacy services at JPAFHC, and telehealth appointments are available to help make patient care more accessible.
