For nearly 40 years, we have proudly served the communities of Southeast Queens and Red Hook, Brooklyn, providing high-quality, community-based healthcare for generations of families. As a Federally Qualified Health Center (FQHC), we are committed to expanding access to coordinated, patient-centered care regardless of a patient’s ability to pay. 

At JPAFHC, we deliver integrated, whole-person care that combines medical, behavioral, dental, and supportive services under one healthcare network. Patients also have access to behavioral health, podiatry, and pharmacy services at JPAFHC, and telehealth appointments are available to help make patient care more accessible.
