Maulik Bhalani, MD, is the founder and CEO of Florida Pain Medicine, an interventional spine and pain management facility with locations in Tampa, Wesley Chapel, Brandon, Zephyrhills, Riverview, Ormond Beach, Port Orange, and St. Petersburg, Florida. He is also the founder and medical director of Surgery Center of Florida. Douglas Constant, MD, is a board-certified pain management physician at the 360 Orthopedic locations in Sarasota and Lakewood Ranch, Florida. He specializes in interventional treatments of the neck and spine and has a special interest in all forms of spinal injections, including epidural and facet injections and ablations. Justin M Raye, DO, is a board-certified physical medicine and rehabilitation (PM&R) physician and interventional pain management specialist at 360 Orthopedics in Sarasota, Venice, and Lakewood Ranch, Florida.

Dr. Raye earned his Bachelor of Science in applied physiology and kinesiology (Summa Cum Laude) at the University of Florida. He then completed his Doctor of Osteopathic Medicine degree at DeBusk College of Osteopathic Medicine in Tennessee, where he graduated in the top 10% of his class. He completed his internship at Advocate Illinois Masonic Medical Center, before completing his PM&R residency at the University of Minnesota, where he was also named Chief Resident. Dr. Raye rounded out his medical training with an interventional pain management fellowship at Arizona Pain Specialists. 

Dr. Raye’s research appears in several medical journals and his clinical trials have proven valuable in stem cell repair and spinal stenosis treatment. He belongs to the American Society of Interventional Pain Physicians, North American Neuromodulation Society, Spine Intervention Society, Academy of Integrative Pain Management, and the American Academy of Physical Medicine and Rehabilitation.
