I’m a Licensed Professional Counselor (LPC) and National Certified Counselor (NCC) with five years of experience working with children, adolescents, and adults. Throughout my career, I’ve had the opportunity to work with people experiencing anxiety, depression, trauma, behavioral concerns, relationship challenges, and major life transitions.

My approach to counseling is warm, supportive, and individualized. I believe therapy works best when you feel comfortable being yourself and know that you are being heard without judgment. I am a Board-certified Family Nurse Practitioner (FNP) licensed in Texas with several years of experience as a health care provider at various levels within an acute and family practice healthcare setting. I am passionate about the use of holistic and integrative medical therapies to address the needs of proactive healthy individuals and individuals with acute or chronic medical conditions across lifespans. I pride myself in utilizing evidence-based models to determine the underlying causes of disease and adapt treatment approaches that include FDA-approved medications, therapeutic levels of innovative IV therapies, and personalized medical weight loss strategies to restore the body to an optimally functional state. My approach to health care focuses on providing excellent care, an adaptation of a proactive rather than reactive healthy lifestyle, and support of long-term health maintenance of the mind, body, and spirit.

A graduate of UT San Antonio, “Dr. J” (as she is known to her patients) joined us in 2004. Dr. Jaiswal heads our care coordination and outreach program that ensures the highest quality of care and customer service. She gets much joy in caring for those who are in need, and is known for her loving and compassionate bedside manners, especially when caring for the elderly. Her clinical emphasis is on managing hypertension and heart disease. Outside of work Dr Jaiswal enjoys spending time with family and friends.

 

Dr. Monica Ramirez is a Dallas native who has just completed her residency at UT Health San Antonio.  She is excited to be back in Dallas and thrilled to be joining Southwest Family Medicine Associates. 

Dr. Ramirez pursued Family Medicine as it allows her to form trusting and long-lasting relationships with patients and their family members. She is a strong believer of the “whole person” approach which is at the heart of family medicine and an advocate of education and preventive medicine.

After completing her residency in 1999 at St. Paul’s Hospital in Dallas, Dr. Dharma single handedly grew her small medical practice into Southwest Family Medicine Associates (SFMA). As the medical director, she brings her passion for innovative and cutting edge technology to the practice, and worked to secure recognition of SFMA as a “medical home” from the National Committee for Quality Assurance (NCQA)-an elite accreditation which only 1,500 clinics in the nation hold. Dr. Dharma has a “no stone left unturned” philosophy to medicine, which has earned her a strong following of patients. She also believes that prevention is the key to good health, which lead her to establish our on-site wellness center in 2014. Her dedication to medicine is represented by the multiple board positions she holds, including president of her local chapter to being on the board of the national Texas of Physicians- (TAFP). As an associate clinical faculty member at UT Southwestern, she has taught medical student for over 15 years. In her spare time, she enjoys cross-fit, Capoeira martial arts, and spending time with ther family.

Dr. Prachi Thapar is a board-certified Family Medicine physician and a DFW native. She grew up in Flower Mound, TX, and graduated from Washington University in St. Louis. She completed her medical school education at the Texas College of Osteopathic Medicine in Fort Worth. Subsequently, Dr. Thapar completed her Family Medicine residency at UT Southwestern in Dallas, where she developed a robust clinical background and a deep understanding of patient care.

She became a Family Medicine doctor as it provided the opportunity to be on the frontline of preventive healthcare, working as a part of a collaborative effort to establish long-lasting relationships with patients of all ages and diverse backgrounds to understand their unique narratives. Her commitment to her patients goes beyond mere treatment; she strives to empower them and to work together towards a shared vision of equitable health and well-being.

Dr. Thapar also has a strong interest in structured healthcare advocacy efforts and is dedicated to addressing health disparities and promoting inclusive care that respects and acknowledges the varied experiences and needs of her patients.

In her free time, Dr. Thapar is an avid dancer and amateur painter. She enjoys reading the latest mystery novels, attempting to solve challenging crossword puzzles, and traveling.

 Sarah Ashitey, MD is board certified in Family Medicine and provides comprehensive care both for the individual and family. She brings a patient-centered personal touch into every encounter and believes that each patient’s unique story is central to the healing process and path to wholeness and wellbeing.

She obtained her medical degree from the University of Texas Southwestern Medical Center in Dallas and completed her residency at the esteemed John Peter Smith Hospital residency program. It is here that she gained the tools needed for broad spectrum family medicine as well as an unparalleled experience in procedural training.

Consequently Dr. Ashitey is able to offer her patients a wide array of services from her skill set including but not limited to chronic disease management, joint injections and dry needling, laser therapy and hormone therapy.

Harman P. She completed her internship at the Internal Medicine Residency Program in Spokane, Washington (an affiliate of the University of Washington School of Medicine in Seattle, WA) and then her Residency at SUNY-Buffalo Internal Medicine Residency Program in Buffalo, NY. Her Fellowship Training in Oncology was at the nationally - acclaimed Wilmot Cancer Institute in Rochester, NY. This experienced was followed with a Faculty Position at Roswell Park Cancer Center in Buffalo, NY before moving to the Dallas area with her family. She has also co-authored various book chapters in medical textbooks which are used in teaching programs across the country.

Dr. Kaur is fluent in English, Hindi, and Punjabi. In her free time, Dr. Kaur enjoys spending time with her family, traveling, and volunteering time at the local Sikh Temple where she focuses on her faith, Sikhism. 

 

About Dr. Lopez

Dr. Jaxel Lopez-Sepulveda is a board-certified family medicine physician serving men, women, and children at Clinica tu Salud in Irving, Texas. Dr. Lopez-Sepulveda is dedicated to providing his patients with compassionate, affordable, and individualized care. He founded Clinica tu Salud in 2017.

Dr. Lopez-Sepulveda was born in Puerto Rico and grew up interested in medicine and helping others. He looked up to his father, Dr. Angel Luis López Ruiz, a well-established surgeon in Puerto Rico.

At a young age, Dr. Lopez-Sepulveda realized his vocation to become a physician. His childhood brimmed with trips to the hospital where he would accompany and observe his father at work.
