Frank Lee, MD, is a family and emergency medicine physician with decades of experience. He enjoys practicing medicine at North Tex MedCare Clinic and Urgent Care, which has convenient locations in Allen, Addison, Fort Worth, and San Antonio, Texas.

Dr. Lee earned his medical degree from the Universidad Autónoma de Guadalajara in Zapopan, Mexico, and New York Medical College in New York City. After graduation, he completed a residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin.

With a passion for urgent care and emergency medicine, Dr. Lee has extensive experience providing responsive care to patients. Mallory Soto is a board-certified family nurse practitioner working in the offices of James E. Race, MD.
She earned her Bachelor of Science in Nursing at Texas Tech University Health Sciences Center and her Master of Science in Nursing as a family nurse practitioner at Chamberlain University, School of Nursing.
Mallory has worked in the Renal Med-Surge unit of UMC Health System in Lubbock, Texas, in Pediatrics as a school nurse with the Ft. Worth Independent School District, and the Cardiology Med-Surge department at Methodist Hospital in Mansfield, Texas.
Mallory has lived in Texas all her life and relocated to Dallas after Graduating from Texas Tech. When Mallory was growing up, she was influenced by her school nurse to get involved in healthcare, and with the love and support of her family, she made it happen.
She is a member of the American Association of Nurse Practitioners (AANP), American Nurses Association (ANA), and Pi Beta Phi Fraternity.

She obtained her Master of Science in Family Nurse Practitioner from Winston-Salem State University in North Carolina and is board certified by the American Association of Nurse Practitioners. Before moving to Waxahachie, Texas she lived in Killeen, Texas to support her military spouse. She worked in the community as a house call nurse practitioner. She also provided primary care as a nurse practitioner in Mansfield, Texas.

Iyen Omoragbon, FNP-C, is an outstanding family nurse practitioner at WithCare Clinic in the Bedford area. He is board-certified by the American Association of Nurse Practitioners and has more than 12 years of medical experience.

Iyen specializes in urgent care and general family medicine. He earned his bachelor’s degree in nursing from Midwestern State University in Wichita Falls, Texas, and obtained his master’s degree as a family nurse practitioner from Walden University, based out of Minneapolis, Minnesota.

After graduating in 1998 from the University of Medical Sciences of Havana, Cuba, I practiced primary and emergency medical care for 8 years. Then I specialized in Otolaryngology and gained the experience from both Cuba and Paraguay for 7 years. Upon arriving in the United States later that year, I began studying nursing with the purpose of becoming a Nurse Practitioner. Then I served as the Allergy Nurse Manager at ENT clinic for nearly two years. I treat patients of all ages, focuses on family medicine, cares for chronic and acute diseases, minor emergencies, allergies and the assesses certain otolaryngology pathologies.My passion is to see my patients smile once they are recovered.

Dr. Ebere Israel Azubuike, MD ("Dr. Izzy"), is a family doctor serving patients of all ages at Mercy Family Clinic in Dallas, Texas. He has about 15 years of experience, and he specializes in primary care and family medicine. 

Dr. Izzy Azubuike graduated from the University of Texas Medical Branch School of Medicine in Galveston, Texas, in 2005. He completed his residency at the Methodist Health System Dallas, Texas, in 2008.

After finishing his formal education, Dr. Azubuike started initially as a hospitalist, seeing patients at various hospitals in Dallas area. During this period he realized that most of his hospital patients do not have a primary care doctor. For this reason, he opened his private practice, Mercy Family Clinic, (M.F.C.) in 2010. His main purpose was to start early preventive care with holistic approach to treatment, hence decreasing the need for these patients to go back to the hospital. Dr. Jaxel Lopez-Sepulveda is a board-certified family medicine physician serving men, women, and children at Clinica tu Salud in Irving, Texas. Dr. Lopez-Sepulveda is dedicated to providing his patients with compassionate, affordable, and individualized care. He founded Clinica tu Salud in 2017.

Dr. Lopez-Sepulveda was born in Puerto Rico and grew up interested in medicine and helping others. He looked up to his father, Dr. Angel Luis López Ruiz, a well-established surgeon in Puerto Rico.

At a young age, Dr. Lopez-Sepulveda realized his vocation to become a physician. His childhood brimmed with trips to the hospital where he would accompany and observe his father at work.

Since his early years, Dr. Lopez-Sepulveda has found the field of medicine riveting and believes the experiences with his father taught him the framework and principles of methodical, empathetic, and efficient care. His primary goal is to enhance patient wellness overall.

 Sarah Ashitey, MD is board certified in Family Medicine and provides comprehensive care both for the individual and family. She brings a patient-centered personal touch into every encounter and believes that each patient’s unique story is central to the healing process and path to wholeness and wellbeing.

She obtained her medical degree from the University of Texas Southwestern Medical Center in Dallas and completed her residency at the esteemed John Peter Smith Hospital residency program. It is here that she gained the tools needed for broad spectrum family medicine as well as an unparalleled experience in procedural training.

Consequently Dr. Ashitey is able to offer her patients a wide array of services from her skill set including but not limited to chronic disease management, joint injections and dry needling, laser therapy and hormone therapy.

She heads up the Sexual Healing and Wellness programs at Southwest Family where we offer Adam and Eve packages using laser and PRP shots for Penile Enhancement and Vaginal Rejuvenation.

Dr. Monica Ramirez is a Dallas native who has just completed her residency at UT Health San Antonio.  She is excited to be back in Dallas and thrilled to be joining Southwest Family Medicine Associates. 

Dr. Ramirez pursued Family Medicine as it allows her to form trusting and long-lasting relationships with patients and their family members. She is a strong believer of the “whole person” approach which is at the heart of family medicine and an advocate of education and preventive medicine.
