Fellowship-trained pain management specialist Miranda Reed Cleaver, DO, provides exceptional services to patients at the Cleaver Medical Group Interventional Pain offices in Dahlonega, Dawsonville, and Cumming, Georgia. Dr. Cleaver grew up in Chatsworth, Georgia, and attended the University of Georgia in Athens. She graduated in 2006 with a bachelor’s degree in biology. After college, Dr. Cleaver went to medical school at the Philadelphia College of Osteopathic Medicine’s Georgia Campus in Suwanee, Georgia. After completing her medical degree, Dr. Cleaver did her residency in anesthesiology at McLaren Oakland Hospital in Pontiac, Michigan.

Dr. Cleaver completed a fellowship in pain management at McLaren Oakland Hospital. During her training, she perfected her skills in interventional pain management procedures. She established a pain management practice serving two hospitals in Northern Missouri. But her goal was always to return to her home state. She achieved that with the creation of the Cleaver Medical Group in Georgia.

Dr. Cleaver’s professional interests include lumbar (lower back) and cervical (neck) pain, particularly conditions caused by damaged spinal discs. Board certified in anesthesiology and interventional pain management, Dr. Miller specializes in spinal cord and dorsal root ganglion stimulation as well as the Vertiflex™ procedure plus she is a member of The Society of Women Innovators in Pain Management, the North American Neuromodulation Society (NANS), and the Georgia Society of Interventional Pain Physicians (GSIPP). Dr. David Neckman practices at the Conyers, Covington, and Piedmont/Atlanta locations.  Dr. Neckman graduated from the Icahn School of Medicine at Mount Sinai in New York City where he also went on to complete a residency in anesthesiology and a pain medicine fellowship. He is double board certified in anesthesiology and pain medicine in addition to being named an Atlanta magazine Top Doctor in 2022, and again in 2023. Dr. Neckman also serves on the Clinical Governance Committee for Piedmont Hospital as the interventional pain management representative and has practiced in the Atlanta area for 5 years.

David Rosenfeld, MD, is an established anesthesiologist at Alliance Spine & Pain, serving patients at their Carrollton, Covington, and Peachtree City, Georgia offices. Passionate about pursuing health care, Dr. Rosenfeld received his medical degree from the University of Wisconsin in Madison. He later completed an internship in General Surgery at LSU Medical Center in New Orleans, Louisiana.

Dr. Rosenfeld then completed his residency in anesthesiology at Alton Ochsner Medical Foundation in New Orleans, Louisiana, where he later completed advanced fellowship training in interventional pain medicine.

Contributing to his vast experience, Dr. Rosenfeld served as the former chief of anesthesiology and the director of the Center for Pain Management at Oktibbeha County Hospital in Starkville, Mississippi. He was previously in private practice in New Orleans, Louisiana, and served on the faculty at Tulane University Medical Center.

Andrew Kilday, MD, is a skilled anesthesiologist and pain management specialist at Alliance Spine & Pain in Lawrenceville, Sandy Springs, and Covington, Georgia. He looks forward to serving patients in and around the community, providing them with excellent care. 

Dr. Kilday graduated from Mercer University in Macon, Georgia, with a degree in business management. Understanding his passion for people, he began his graduate studies at the University of Georgia, eventually earning his master’s in Public Health Policy and Management. 

Taking his interest in healthcare globally, Dr. Kilday received his medical training at the University of Queensland’s Ochsner Medical School in Brisbane, Australia, and New Orleans, Louisiana. Relocating to Atlanta, Georgia, Dr. Kilday completed both a residency in anesthesiology and a fellowship in interventional pain management at Emory University’s School of Medicine.

Richard Gayles, MD, is a double board-certified specialist in anesthesiology and pain management and a nationally recognized leader in regenerative medicine. With a strong commitment to excellence and patient-centered care, Dr. Gayles practices at both the Piedmont/Atlanta and Douglasville, Georgia, locations of Alliance Spine & Pain.

Upon receiving a Bachelor of Science in Psychobiology from the University of Michigan, Dr. Gayles went on to complete his Doctorate of Medicine at the University of Michigan's School of Medicine.

Dr. Gayles furthered his training by completing an anesthesia residency at Johns Hopkins in Baltimore, Maryland, and a specialized pain management fellowship at The Cleveland Clinic in Ohio. Prior to his fellowship, Dr. Gayles' dedication to learning and expanding his horizons led him overseas to England, where he spent a year studying anesthesiology and critical care at the prestigious St. Bartholomew's Hospital in London as a visiting registrar.

After earning his Doctorate in Medicine at Albany Medical College in Albany, New York, Dr. Okpareke then completed an anesthesiology residency at the University of Chicago followed by a fellowship in chronic interventional pain management at the prestigious Rush University Medical Center in Chicago. Dr. Okpareke then spent eight years practicing as an interventional pain specialist in Chicago before returning to Atlanta in 2022 to join Alliance Spine & Pain where he now serves on its Medical Executive Committee.
