At ChronosCare, we are dedicated to delivering compassionate, high-quality geriatric care to adults aged 65 and older across the Los Angeles area.  Our innovative hybrid care model seamlessly combines telehealth and in-home medical services, tailored specifically to meet the needs of those residing in assisted living communities. Led by Raffi Hodikian, MD, a board-certified internal medicine physician with extensive experience in geriatric and palliative care, ChronosCare takes a comprehensive, patient-centered approach—addressing not only medical conditions but also supporting overall well-being, independence, and quality of life at every stage of aging. 

To ensure care is as accessible and seamless as possible, ChronosCare offers a comprehensive range of medical services through a secure telehealth platform, easily accessible via smartphone, tablet, or computer. For patients who require in-person care, Dr. Hodikian provides home visits for physical examinations and other essential evaluations, bringing high-quality medical care directly to the comfort of the patient’s home. Families are also supported through real-time teleconferencing, allowing them to participate actively in their loved one's care and stay informed every step of the way.  

Our multi-specialty group provides a broad range of services, including primary care, podiatry, endocrinology, and neurology, all tailored to meet the unique needs of older adults. To ensure continuity and coordination of care, all patient information is managed through a centralized electronic medical record system, offering seamless communication and visibility across specialties. This integrated approach enhances the quality, safety, and efficiency of care. Additionally, we place a strong emphasis on preventive health, offering services such as memory screenings, incontinence evaluations, and fall risk assessments to support independence and reduce avoidable complications. For patients managing multiple prescriptions, our comprehensive medication management program includes personalized oversight and educational support for both patients and caregivers—promoting safe, effective, and informed treatment decisions.
