I’m Kelli Kraaz, APRN, and as an advanced practice registered nurse, I work to ensure you receive the quality health care you need from your home, office, or wherever is most convenient.
My career spans 35 years, serving in hospitals, clinics, and trauma settings. I have extensive experience making rounds, admitting and discharging patients, and developing personalized treatment protocols. My background has given me the skill set to handle everything from acute and chronic disease management to wound care, preventive care, and more.
As an autonomous nurse practitioner, I function independently of physicians but regularly collaborate with various health specialists. I visit patients' homes, offices, and assisted living facilities to diagnose and treat various medical issues, including asthma and chronic obstructive pulmonary disease (COPD), hypertension (high blood pressure), and diabetes. I also provide telehealth visits.
Though I don’t maintain a physical office, I can still use cutting-edge technology, including digital stethoscopes and electrocardiograms (EKGs), that allows me to listen wirelessly, record, playback, and save. I perform most services onsite, but when necessary, I can refer you to nearby clinics and care facilities.
I offer a conservative, patient-centered approach, prescribing alternative and holistic therapies whenever possible. I never rush appointments and dedicate intentional time to learning about your symptoms, concerns, and goals during appointments.

Logan T. Vander Woude, DO, MPH, is a board-certified ophthalmologist who is fellowship trained in cataract, cornea, refractive and glaucoma surgery.
Dr. Vander Woude graduated as valedictorian of his medical school class at Nova Southeastern University in Fort Lauderdale, FL, where he was given the Dean’s Award and was inducted into the Gold Humanism Honor Society. He completed his three-year ophthalmology residency at University of Florida in Gainesville, during which he was elected as chief resident. He then completed a one-year dual fellowship in cornea, refractive and glaucoma surgery at Your Eye Specialists in Plantation, FL.
Born and raised in Green Bay, Wisconsin, Dr. Vander Woude completed his undergraduate degree at University of Wisconsin-Madison. After which, he completed a Masters in Public Health and Fellowship in Global Health, leading projects in Nepal, India, Ghana, and here in south Florida.

Frank Ditz, MD, is a skilled and experienced family practice physician with a passion for personalized care. Through his work at Family Practice of Suntree and Viera in Melbourne, Florida, Dr. Ditz is honored to play a role in helping those in his care to improve their health and wellness.
Education has always been an important part of Dr. Ditz’s life. He began his studies at the State University of New York at Buffalo, where he attended on an academic scholarship and graduated with honors. He then focused on earning his medical degree from the Uniformed Services University of the Health Sciences in Bethesda, Maryland.
Upon graduating and receiving his commission as an officer in the United States Air Force, Dr. Ditz completed his internship and residency in family practice at David Grant USAF Medical Center and the University of California, Davis, School of Medicine. He is proud of his 14 years of service in the United States Air Force.
While serving, Dr. Ditz was selected to study at the School of Aerospace Medicine in the role of flight surgeon. He was one of only a few physicians certified to go through 9G training required to fly in any type of fighter aircraft.
Despite receiving numerous attractive offers, including the opportunity to rise to the level of Lieutenant Colonel, Dr. Ditz left the military to pursue his dream of going into private practice. Over more than two decades in practice, Dr. Ditz has developed expertise in many areas of practice. Kali Garcia Malpica (ARNP) one of the founders and Clinical Administrator was born in Matanzas, Cuba. In 2011 her husband and daughter immigrated to the United States. She is fluent in both Spanish and English. Kali attended School of Medicine in Cuba and received her Medical Degree in 2005. In November 2007 she completed the internship and residency program in Family Medicine at University of Matanzas. In July 2014 she completed her Bachelor of Science in Nursing and her Master of Science degree at South University in Savannah Georgia. She holds a national board certification with the American Academy of Nurse Practitioners (AFNP) as a family nurse practitioner. Kali Garcia Mapica (ARNP) is affiliated to Cleveland Clinic Hospital.
