Mona Clor, MD, is a highly qualified Primary Care Physician and Urgent Care physician at Michigan Avenue Primary Care, serving patients throughout The Loop of downtown Chicago, Illinois.

Anne-Sophie Kim, MD, is a dedicated primary care physician at Michigan Avenue Primary Care located in The Loop of downtown Chicago, Illinois. She is committed to patient-centered medicine and provides comprehensive and preventative primary care.

Dr. Jeannette Kelly is a dedicated family physician, working in association with Advocate Illinois Masonic Medical Center. She is a reputed alumna of Michigan State University, where she received her medical training and her bachelor’s degree. She completed a residency in family medicine at West Suburban Medical Center and advanced her clinical abilities in primary care sports medicine with a fellowship from MacNeal Hospital.

Dr. Kelly is certified by the American Osteopathic Board of Family Physicians and the American Board of Family Medicine with a sub-specialization in sports medicine. Dr. Barrett attended Virginia Tech for his undergraduate degree, studying biochemistry. He earned his medical degree at Eastern Virginia Medical School and completed his residency at the University of Illinois at Chicago.

Earning her medical degree from the University of Munich, Dr. Lengyel pursued further training in obstetrics and gynecology while in Germany before moving to the United States to work as a postdoctoral research fellow at the University of California San Francisco.  After publishing multiple articles and book chapters, Dr. Lengyel returned to clinical medicine and completed her Family Medicine residency at West Suburban Medical Center.

Brett is an integrative nurse practitioner who is board certified in both internal medicine and sexual/reproductive medicine. Prior to joining Whole Health Chicago, Brett worked in OBGYN clinics where she was able to treat a variety of conditions such as PCOS, endometriosis, uterine fibroids, PMS, PMDD, depression, anxiety, and perimenopausal/menopausal symptoms. She is trained in contraceptive management, including IUD/Nexplanon insertions and removals. Since becoming a Family Nurse Practitioner in 2016, she has worked at Federally Qualified Healthcare Centers located in the heart of Chicago’s Southeast and Westside neighborhoods, where she primarily served underprivileged populations. Keisha brings a breadth of experience as a primary care provider and also as a Registered Nurse working more than 14 years caring for patients of all ages in the areas of Medical-Surgical nursing, Labor and Delivery, managed care, and nursing administration at several Chicagoland area hospitals. Keisha also teaches and mentors Family Nurse Practitioner students at the University of Illinois at Chicago. Keisha has a genuine passion for providing holistic community healthcare.

Jaclyn (Jackie) is a Speech-Language Pathologist who earned her Master’s degree at Midwestern University in Speech-Language Pathology. Jackie has been practicing as a speech-language pathologist at EYT since 2018. Some recent life changes have brought Jackie out of state, and she is now primarily seeing children via telehealth for the IL and MD branch.  

 Jackie believes in engaging children in play-based activities to foster overall language development. Jackie is certified in PROMPT Therapy,  DIRFloortime® Certified, and has training in Interactive Metronome. Jackie specializes in children with Apraxia of Speech, pragmatic language disorders, and a variety of expressive/receptive language disorders.
