Jeff studied at Florida State University, where he received a Bachelor of Science in Political Science and Communications. Upon graduation, he attended Florida Atlantic University graduating Magna Cum Laude in the BSN program. 

Jeff worked over six years in a Trauma 1 hospital located in Fort Lauderdale, providing acute care to hundreds of patients. One aspect he disliked about nursing was that once the patient was discharged from the hospital, the nurse no longer participated in the plan of care. 

So Jeff elected to advance his formal education and pursue a Master’s degree in the Family Nurse Practitioner program at South University, where he graduated with High Honors. Becoming a Family Nurse Practitioner provided him the opportunity to have a more primary role in the care of his patients. 

As a primary care provider, he has been able to deliver quality healthcare to families throughout their life span and health continuum. His plan of care objectives for his patients are based on health promotion and disease prevention.

Having grown up in South Florida, Jeff continues to stay active in many community organizations. During his free time, he plays multiple sports and attends the gym daily. He is an avid sports fan of many Florida teams, including the Florida State Seminoles, the Miami Dolphins, and the Miami Heat.

Leandro Rojas, APRN, is an experienced family nurse practitioner who provides exceptional patient care at AssociatesMD in Pembroke Pines, Florida. 

Along with his general practice and family medicine training, Leandro has specialized skills in ophthalmology and psychiatry, allowing him to provide the best complete care to his patients. 

Leandro completed his nursing and advanced practice nursing education at Miami Regional University in Miami Springs, Florida. He then spent about ten years in Venezuela as a practicing nurse before returning to Florida in 2014. He started serving his local community through crisis health services at that time.

Dr. Peralta grew up in South Florida and graduated from McArthur High School in Hollywood, Florida. She earned her nursing degree at Texas Christian University in Fort Worth, Texas, and then returned to her home state of Florida to work as an emergency room nurse before starting medical school. 

Dr. Peralta earned her medical degree at Nova Southeastern University (NSU) College of Osteopathic Medicine in Davie, Florida. She’s an active member of the American Osteopathic Association (AOA) and the Florida Osteopathic Medical Association (FOMA). 

Dr. Peralta is bilingual and converses with patients in English or Spanish, whichever makes them the most comfortable. She is currently treating patients in the Weston, Florida, office at AssociatesMD, and welcomes patients to make telemedicine appointments.

Ms. Carlisle is a graduate of Florida Atlantic University in Boca Raton, where she earned her Bachelor of Science in Nursing, Summa Cum Laude. Certified by the American Nurses Credentialing Center, she has worked as an ARNP for over eight years, of which five were in the Emergency Department. 

After gaining experience as a nurse, she decided to further her career by pursuing Advanced Practice at her alma mater, Florida Atlantic University. This program let Ms. Carlisle expand her scope in the medical field and treat patients in a new role.

Cindy Katanick, DO, is a board-certified osteopathic family medicine provider at AssociatesMD in Plantation, Florida. Born and raised in Detroit, Michigan, Dr. Katanick watched the “Marcus Welby, MD” television series as a kid and knew then that she wanted to become a doctor.

Dr. Katanick began her career by attending Michigan State University for her undergraduate studies. Relocating to Madison Heights, Michigan, Dr. Katanick completed her internship at the Oakland General Hospital. Melinda Exume, MD, is an experienced primary care physician. She treats patients at AssociatesMD from their Margate, Florida, location. 

Dr. Exume completed her Bachelor of Science in biology and later earned her medical degree at the University of Florida, College of Medicine, in Gainesville, where she graduated with honors in research. She completed her internal medicine residency at the University of Florida Health (UF Health) in Jacksonville, where she honed her clinical skills. After completing her residency, she worked as a hospitalist, efficiently and effectively managing acute care patients. 

Advent Health awarded Dr. Exume “Excellence in Patient Satisfaction, Inpatient Top Performing Hospitalist” in 2020. In 2022, she transitioned to outpatient work with AMD to focus on a more holistic and preventative approach to medicine.

Dr. Exume is of Haitian descent and is fluent in Haitian Creole. Serving on medical mission trips to Nicaragua, Dominican Republic, and Peru laid the groundwork for her acquisition of Spanish as a third language. She believes that communication is the most important aspect of patient care and, therefore, actively strives to continue improving her language and communication skills.

Joelle Estrada, MD, is a dedicated internal medicine physician with nearly two decades in practice. At AssociatesMD, Dr. Estrada provides patient-centered internal medicine in the Weston, Florida, office. 

Dr. Estrada was born in Manhattan, New York, and raised in Paraguay. She earned her medical degree at Universidad Nacional de Asunción in Asunción, Paraguay. 

Dr. Estrada completed her three-year internal medicine residency at Wyckoff Heights Medical Center in Brooklyn, New York. She stays up-to-date with her medical education to make sure her patients always receive optimal care.

Dr. Reines is board-certified in family practice by the American Board of Family Medicine, and he welcomes patients from all age groups. 

Dr. Reines earned his medical degree at Tufts University School of Medicine in Boston, Massachusetts. He went on to complete his internship and residency at Jackson Memorial Hospital, the main teaching hospital for the University of Miami in Miami, Florida.
