Dr. Jerry Neria MD, MPH is well trained with an extensive history and passion for health promotion and lifestyle medicine. Dr. Neria is a firm believer in the power of lifestyle changes over a prescription.
Dr. Neria's passion for diet and lifestyle stemmed from his history as a competitive teenage bodybuilder. He began serving his patients as a personal trainer while obtaining a degree in Kinesiology at the University of Southern California. From there he went on to incorporate nutrition as he began Need2Train LLC, for solo and group personal training along with co-founding the nutrition store, CarbZero. He obtained his Master's degree in Public Health Nutrition, from Loma Linda University. After completing medical school, Dr. Neria returned to Loma Linda to complete his residency training in rural and Family Medicine.
After completing residency, Dr. Neria became a full-time hospitalist and Family Medicine Residency faculty member for over 5 years. He led the residency as the Program Director of the Hanford Family medicine Program and moved on to help found the Tulare Family Medicine Residency Program. Dr. Neria still maintains an interest in education and teaching future doctors.
He is currently serving Living Water Clinic patients at all the clinic sites and maintains an interest in lifestyle medicine, weight loss and obesity medicine, sports nutrition, and hormone replacement.

Nurse Practitioner Michele Millan-Patterson, AGNP-C, welcomes new patients at Living Water Clinic’s Visalia and Porterville locations.
A Central Valley native, she was born and raised in Lindsay, California. After earning a Bachelor of Social Work at Fresno State, she developed community programs—from Latino mental health services to hospital outreach. Working closely with patients and families inspired her to provide care at the bedside.
She returned to school at College of the Sequoias to become a Registered Nurse. She gained experience in critical-care settings, including the ICU, cardiac ICU, emergency department, and medical-surgical units. She also learned clinic administration, but her passion remained direct patient care. That calling led her to complete Nurse Practitioner training and earn board certification as an Adult-Gerontology Nurse Practitioner (AGNP-C).
Michele provides comprehensive primary care for adults and seniors. Her services include wellness exams, routine checkups, and chronic condition management for diabetes and hypertension. She offers preventive care such as immunizations and screening tests, women’s health and family planning support, and acute care for minor injuries and sudden illness. She collaborates with specialists, health educators, and community resources to provide whole-person support.
In every visit, Michele listens carefully, explains recommendations in clear terms, and involves each patient in decision-making. She respects that patients are experts on their own lives. Her role is to guide, educate, and support so that each person feels confident in their health.
Outside the clinic, she enjoys traveling with her husband and two sons, lifting weights, gardening, sewing, reading, and hosting friends for home-cooked meals. Staying active and curious helps her bring energy and empathy to every patient visit.

Leslee Heuer, MS, FNP-C, is a valued and trusted certified family nurse practitioner at Living Water Community Health and Wellness Clinic, serving Porterville, Lindsay, and Visalia, California. She specializes in diagnosing and treating a wide range of chronic and acute health concerns that affect children and adults. Mrs. Heuer attended California State University, Fresno, where she earned her Bachelor of Science in Nursing and Health Administration. Throughout the nursing program, Mrs. Heuer remained dedicated to her studies, earning the distinction of graduating cum laude. In addition, Mrs. Heuer was a member of the Golden Key Honor Society, Phi Kappa Phi, and Sigma Theta Tau.
After gaining experience working in several different hospital settings as a nurse, Mrs. Heuer decided to continue her education. She attended the University of Phoenix, where she earned her Master of Science in Nursing in 2009. Since then, she has worked as a nurse practitioner in several clinics throughout the San Joaquin Valley before joining the team at Living Water Community Health and Wellness Clinic.
Mrs. Heuer is a member of the American Academy of Nurse Practitioners as well as the Council for Affordable Quality Healthcare.
