I was born in Cuba and later emigrated to Spain before coming to the U.S.. I trained in Belize at St. Matthew’s School of Medicine and then in Manchester, England. My residency took place at the University of Birmingham Alabama for Family Medicine Specialty. I became Board Certified in Family Medicine in 2005. I have spent my majority of practice in Emergency Medicine. I have numerous certifications with extensive continuing medical education. I am proficient in multiple procedures from colposcopy to colonoscopy, along with a variety of up to date skin procedures.
I was born in Cuba and have a Siamese twin sister who is also a medical physician. I trained in Belize at St. Matthew’s School of Medicine and also in Manchester, England. I graduated from UAB Family Residency program in 2006. I have been practicing in Florida for 13 years as an emergency room physician. I am board certified in family medicine with multiple accreditation's such as ACLS, BLS, PALS, NALS, and ATLS. I believe continuing medical education should be done constantly to stay up to date with all new research.

Dr. Nelson is board certified in Family Medicine and Osteopathic Manipulative Treatment (OMT) by the the American Osteopathic Association upon the recommendation of the American Osteopathic Board of Family Physicians. He completed his medical degree at Lincoln Memorial University DeBusk College of Osteopathic Medicine in Harrogate, TN. After medical school, Dr. Nelson completed his residency training in Family Medicine at Largo Medical Center, Largo, FL.
Before joining APC Health, Dr. Nelson served in the United States Air Force as Staff Family Physician and is now a proud Veteran of the United States Armed Forces in good standing.

Sergio B. Seoane, MD, is an experienced family medicine physician with a private practice in Lakeland, Florida. He serves patients over 18, is fluent in English and Spanish, and is skilled in all areas of family medicine.

Janelle Bauer is a board-certified family nurse practitioner at New Tampa Interventional Pain and Sport Medicine. Janelle completed her Master of Science in Nursing with a family nurse practitioner specialty from the University of South Florida and is dual certified by both the American Nurses Credentialing Center and the American Association of Nurse Practitioners. However, her healthcare journey began as a certified nursing assistant directly out of high school. Her family instilled in her the importance of a strong work ethic. She worked throughout college to obtain a Bachelor of Science in Nursing at the University of South Florida. As a registered nurse, she specialized in the surgical intensive care unit at Lakeland Regional Health. She began working at New Tampa Interventional Pain and Sports Medicine in 2021. In her free time, Janelle enjoys painting, gardening, and getting outdoors with her family. She has 2 young sons who are active in soccer and baseball.

Dr. Jewel L. Tartt, APRN, DNP is a certified Nurse Practitioner with a Doctor of Nursing Practice who specializes in Family Medicine. As a Primary Care Provider, Dr. Tartt has extensive experience educating Patients on preventative care and prescribed treatments, and collaborating with Care Teams in assessing, planning, and implementing the Patient's care plan.
Dr. Tartt obtained her Family Nurse Practitioner degree from Duquesne University School of Nursing, where she later continued to obtain her Doctor of Nursing Practice. As a Primary Care Provider Dr. Tartt sees Patients in their homes, where the visit becomes part of their assessment for lifestyle and total well-being.
Dr. Tartt believes in-home visits provide a comfortable, familiar environment where she and her Patient can work together for their health and their life. She started Holistic Direct Primary Care out of a passion and advocacy for her Patients.
