About Dr. Nomen Azeem
Dr. Azeem completed his undergraduate education at James Madison University in Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident.
After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Falco.
Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida. He recently developed the first successful non-opioid based interventional pain clinic in the Sarasota, Florida, area.
His background helped shape Dr. Azeem’s approach to treating pain, and he includes both patients and their families in developing treatment plans so that everyone involved understands the origin of the patient’s pain as well as their available treatment options.
Currently, Dr. Azeem is an associate assistant professor in the pain/neurology department of the University of South Florida, and actively continues to conduct research and author peer-reviewed publications in addition to treating patients, President, Florida Society of Pain & Neuroscience, Chair of Strategic Development for State Society Engagement and Advocacy, American Society of Pain & Neuroscience and Board of Directors, Florida Prescription Drug Monitoring Foundation.

Justin M Raye, DO, is a board-certified physical medicine and rehabilitation (PM&R) physician and interventional pain management specialist at 360 Orthopedics in Sarasota, Venice, and Lakewood Ranch, Florida.
Dr. Raye earned his Bachelor of Science in applied physiology and kinesiology (Summa Cum Laude) at the University of Florida. He then completed his Doctor of Osteopathic Medicine degree at DeBusk College of Osteopathic Medicine in Tennessee, where he graduated in the top 10% of his class. He completed his internship at Advocate Illinois Masonic Medical Center, before completing his PM&R residency at the University of Minnesota, where he was also named Chief Resident. Dr. Raye rounded out his medical training with an interventional pain management fellowship at Arizona Pain Specialists.
Dr. Raye’s research appears in several medical journals and his clinical trials have proven valuable in stem cell repair and spinal stenosis treatment. He belongs to the American Society of Interventional Pain Physicians, North American Neuromodulation Society, Spine Intervention Society, Academy of Integrative Pain Management, and the American Academy of Physical Medicine and Rehabilitation.
Throughout his medical training, Dr. Raye has provided medical coverage at the USA Cup Soccer Tournament, Twin Cities Marathon, Susan Komen 3-Day Event, and the NCAA Division II men’s soccer team at Lincoln Memorial University in Tennessee.
Dr. Raye is a native of Bradenton, returning to his hometown to give back to our community. During his free time, he enjoys soccer, football, basketball, biking, fishing, boating, the beach, traveling with family and friends, and, of course, Florida Gator athletics.

Douglas Constant, MD, is a board-certified pain management physician at the 360 Orthopedic locations in Sarasota, Lakewood Ranch, and Venice, Florida. He specializes in interventional treatments of the neck and spine and has a special interest in all forms of spinal injections, including epidural and facet injections and ablations, Vertiflex and MILD procedures for spinal stenosis, spinal cord stimulation, peripheral nerve stimulation, vertebral compression, and pelvic fractures as well as the treatment of chronic migraine headaches with Botox therapy.

Paul H. Lento, MD, is a triple board-certified physical medicine and rehabilitation (PM&R) physician and specialist on sports medicine and musculoskeletal ultrasound at 360 Orthopedics in Sarasota, Venice, and Lakewood Ranch, Florida. Over the past several years, Dr. Lento has been named one of the Top Doctors in Sarasota for his ongoing dedication and service.
Dr. Lento attended West Chester University, where he earned his Bachelor of Science in bio-chemistry. He then earned his medical degree from Temple University School of Medicine in Philadelphia before completing an internship via a Navy Scholarship Program at the Naval Hospital of Pensacola in Florida. He then completed a physical medicine and rehabilitation residency at Temple University Hospital, followed by a sports medicine fellowship at the Rehabilitation Institute of Chicago/Northwestern University.
Following his medical training, Dr. Lento was a medical officer for the U.S. Navy. Dr. Lento specializes in the non-surgical treatment of various musculoskeletal problems by identifying the cause of musculoskeletal pain or loss of function and determines whether the problem is mechanical, inflammatory, or degenerative. Together with a variety of non-surgical strategies, Dr. Lento addresses these problems, which enable patients to get back to enjoying life.
His specialty also incorporates the use of Musculoskeletal Ultrasound (MSK US). MSK US is a non-invasive imaging test used to diagnose tendon, muscle, and ligament tears without the use of ionizing radiation or concern for claustrophobia that is associated with MRI. There are also no issues with metal such as pacemakers or other implantable devices. Dr. Lento uses this imaging technique when necessary to accurately and precisely guide injections directly into problematic areas.
Dr. Lento was formerly an advisor for the North American Spine Society providing the appropriate use of exercise to treat various spinal conditions. His practice focus is on identifying biomechanical factors, which may be the root cause of a patient’s musculoskeletal problem.
In 2012, Dr. Lento served as a physician in the Olympic Clinic at the Salt Lake Winter Olympics. He has also covered multiple endurance racing events such as the Chicago, Houston, and Washington, DC marathons. He was Race Medical Director for the International Triathlon Union held at Nathan Benderson Park and currently serves the team physician for Lakewood Ranch, Parrish, and Booker High School athletes.
