She earned her bachelor’s degree in nursing from Oakland University in Rochester Hills, Michigan. Marlene continued on to Madonna University in Livonia, Michigan, where she earned her master’s degree in nursing with advanced training for the full scope of nurse practitioner. She groomed specialized skills in pain management while serving as a pre-operative/recovery nurse in diverse clinical settings, such as surgical intensive care and trauma units. Previously, Marlene worked for the American Surgical Centers and William Beaumont Hospital. She brings her passion for patient care and her commitment to helping patients maintain their health to her work as a member of the Neuro Pain Consultants medical staff.
Lisa M. Provenzano, MSN, RN, NP, is a certified adult nurse practitioner and clinical nurse specialist at Neuro Pain Consultants with four convenient locations in the metropolitan Detroit region: Bloomfield Hills, Clarkston, Macomb Township, and West Bloomfield, Michigan. Lisa shares her well-honed nursing skills with her patients at Neuro Pain Consultants as well as her nursing students at Oakland Community College, where she serves as a member of the adjunct nursing faculty.
Lisa earned her Bachelor of Science degree in nursing from Saginaw Valley State University in University Center, Michigan. She completed her education at Wayne State University in Detroit, where she earned a Master of Science degree in nursing.
For more than three decades, Lisa has served patients in both inpatient and office settings, delivering high-quality patient care as well as health education and disease prevention. Supak Sookkasikon, MD, is an experienced physician and medical director of Emcura Medical in West Bloomfield, Bloomfield Hills, Bloomfield Township, Northville and Grosse Pointe Woods, MI and has been prcticing for over 20 years.
Born in the San Francisco Bay Area, Dr. Sookkasikon earned her Bachelor of Science in Microbiology from the University of California, Davis, before attending medical school in Cambridge, England at the Kigezi School of Medicine. She then returned to the United States to complete her family medicine residency at the University of Michigan. Dr. Sookkasikon is certified by the American Board of Family Medicine and the American Board of Holistic and Integrative Medicine. She’s also certified as a medical review officer and is fluent in Thai.
Dr. Sookkasikon’s commitment to her patients and family throughout her medical training led her to pursue a fellowship in integrative medicine at the University of Arizona Center for Integrative Medicine that was created by Dr. Andrew Weil.

Chaka Chipawe, NP-C, is a board-certified nurse practitioner at White Lake Family Medicine who partners closely with Dr. David Leszkowitz to serve the community of White Lake, Michigan. He treats all types of addictive disorders and specializes in the assessment and treatment of all addictions. His professional expertise extends to advocacy for his patients before insurance and other agencies to ensure approval and provision of necessary tools for satisfactory recovery.
He attended Western Michigan University in Kalamazoo, Michigan, where he received a Bachelor of Science in nursing. Mr. Chipawe then further pursued his studies at Wayne State University in Detroit, Michigan, where he graduated with his Master of Science in Nursing, which qualified him as a Certified Nurse Practitioner in the state of Michigan. While working in the multi-specialty, select specialty, long-term acute care hospital unit, he completed his master’s degree with a subspecialty in adult gerontology care.
Mr. Chipawe began working at White Lake Family Medicine with Dr. Leszkowitz in 2016. He is involved in the entire continuum of care of patients, and you will see him in the office and occasionally the hospital. His passion is in the office, where, along with Dr. Dave, their primary purpose is health promotion and prevention with the goal of preventing his patient population from needing hospital visits.
Mr. Chipawe is an active member of several professional organizations, including the American Society of Addiction Medicine, which has fueled his forever-growing passion for the prevention of fatalities from drug overdose. He works with patients already in recovery and those who have the same goal of reaching recovery, providing long-term support for those who suffer from this disease. Alongside Dr. Leszkowitz, Mr. Chipawe has managed the treatment of people with co-occurring disorders.
Ashley Ziebarth, NP-C is a certified family nurse practitioner at White Lake Family Medicine. As a family nurse practitioner, she has been trained to treat the whole family from infancy to the elderly.
Ashley received her Bachelor of Science in nursing at Oakland University in 2015. She began her career as a bedside registered nurse at Beaumont Hospital in the medical surgical unit. During this time, she monitored both acute and chronic illnesses. She transitioned to charge nurse in a mother-baby unit caring for both newborns and postpartum mothers in women’s health. She furthered her education and completed her master’s degree in nursing at South University. While completing her master’s degree, she received extensive training in women’s health and pediatrics, including routine visits, annual examinations, and preventative wellness.
Ashley is a member of the American Society of Addiction Medicine. She received specific training in this subspecialty for treating and monitoring patients with the disease of opioid and alcohol addiction. Her passion is caring for her patients’ physical and mental health throughout their lifespan.
It has been a great pleasure for her to work alongside Dr. Leszkowitz and Mr. Chipawe. Ashley enjoys helping patients with their addictions and mental health and continuing on their path of recovery. She believes that healing and wholeness are matters of the mind, body, and spirit. She focuses on teaching patients about maintaining a balance for optimal health. She works to achieve a patient-centered approach to disease prevention, management, education, and personalized plans for each patient.
David Leszkowitz, DO, is board certified in addiction medicine and family medicine and is one of just a handful of physicians with these credentials in the state of Michigan. As the founder of White Lake Family Medicine, Dr. Leszkowitz has served the community of White Lake, Michigan, for over 30 years. He has advised many over the years and helped all those who came before him. He effortlessly fights for his patients’ well-being on many fronts. He is held in high esteem by his colleagues, medical educators, insurance companies, and the hospitals for which he advises, and advocates tirelessly on behalf of his patients. His past and present positions as chief of hospital medical departments, director of in- and outpatient substance use disorder clinics, medical educator and clinical faculty at major medical schools, and as a lecturer training other physicians in the field of addiction have proven to be extremely important in helping those patients most in need. Dr. Leszkowitz is one of just four mentors of addiction medicine to other physicians in the state of Michigan. This is through PCSS (Provider Clinical Support Services) with the American College Of Addiction Psychiatry. Dr. Leszkowitz is just one of seven physicians in the state of Michigan credentialed through the American Society Of Addiction Medicine to teach other physicians to be certified in practicing addiction medicine. Dr. Leszkowitz practices the teachings from his medical education, and he holds dear the oath he took upon graduating from medical school: “Do no harm. Do not judge. Accept all that comes.” He teaches other young students and resident physicians the very same principles.
