Milton Chávez, MD, is a board-certified family medicine physician at his practice, Dr. Milton Chávez MD, SC in Wicker Park, Chicago. Dr. Lawrence Kacmar, MD

Dr. Kacmar is an independent family physician who is board-certified in both primary care and sports medicine. A former collegiate football athlete and active adult, Dr. Kacmar recognizes the value of preventing and treating sports injuries from the young school age athlete through the aging adult. With a focus in primary care, Dr. Kacmar is able to care for all ages from routine medical care, preventative exam or acute care needs. Dr. Kacmar remains an admitting physician at Edward Hospital.

Dr. Kacmar's practice interests include orthopedic injuries, critical illness, acute illnesses, acute injury, and patients who are chronically ill seeking a diagnosis. Shannon Keith, RN-MSN, MHA

Shannon Keith joined The Center for Primary Care and Sports Medicine in 2012. Shannon has enjoyed running for most of her life and continues to participate in long distance running. Shannon's practice interests include athlete functional training, weight management, preventative medicine, diabetes and inflammatory disorders for all gender and ages, as well as, perimenopause, menopause and post menopause women conditions. Shannon believes in a holistic approach to medicine and utilizing nutrition, exercise and supplements alone or in combination with pharmaceuticals. 

Shannon holds an undergraduate and graduate degree in Health Care Administration from Grand Canyon University and Master of Science in Nursing from Marquette University. She continues to support education and the development of future nurses as an adjunct professor at Aurora University. 

Mark is board certified as a family nurse practitioner since 2015. Prior to Whole Health Chicago he worked as an APN serving the refugee and immigrant community as well as patients from Chicagoland area. His career in healthcare is rich with experiences including work with nursing home patients, home care, medical mission trips to the Dominican Republic and direct patient care as a medical surgical/float staff RN for 10 years. Mark brings expertise in culturally competent care and proficiency in working with patients and families who are in need of assistance in navigating the healthcare world. Through his experience he has been able to incorporate techniques that are considerate of cultural norms while adapting to the educational and social needs of his clients. He is passionate about treating the whole patient and taking into consideration all aspects of their wellness including mental health, nutrition, disease prevention and lifestyle modifications.

Brett is an integrative nurse practitioner who is board certified in both internal medicine and sexual/reproductive medicine. Prior to joining Whole Health Chicago, Brett worked in OBGYN clinics where she was able to treat a variety of conditions such as PCOS, endometriosis, uterine fibroids, PMS, PMDD, depression, anxiety, and perimenopausal/menopausal symptoms. She is trained in contraceptive management, including IUD/Nexplanon insertions and removals. Doctor Cvengros is a Family Medicine physician with a Certificate of Added Qualifications in Sports Medicine. She has special interest in a healthy diet, exercise, and lifestyle to improve and enhance health. She has attended many nutritional conferences focusing on health and nutrition including the Physicians Committee For Responsible Medicine (PCRM) and the Plantrician Project. She shares a private practice with Doctor Gilberto Munoz at Chicago Med Group/ Munoz MD & Cvengros MD, SC.
