Dr. Azeem completed his undergraduate education at James Madison University in Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident.
After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida. He attended medical school at the University of Cincinnati, where he was appointed as the Cincinnati Academy of Medicine Liaison and Legislative Representative. He was awarded the Golden Scalpel Award for anatomical expertise, and earned his MD in 2002. In 2008, he completed his residency in Anesthesiology at SUNY Buffalo, during which his training was interrupted while he served a tour of duty for Operation Iraqi Freedom.
After residency, Dr. Warren practiced as an anesthesiologist for over 10 years. During this time, he held several key leadership positions, including Chair of Anesthesiology at Doctors Hospital of Michigan in Detroit, and served on the Pharmacy & Therapeutics Committee at Town & Country Hospital in Tampa. He also completed the Baycare Physician Leadership Course while working at St Joseph’s Hospitals.
His commitment to duty has always been an important part of his life. He served for over 15 years in the US Army Reserve, both enlisted and as a medical officer, having served two tours of duty in Iraq.
After practicing anesthesia for over 10 years, Dr. Warren completed an ACGME-accredited fellowship in Interventional Pain Medicine at the University of South Florida. Dr. Kaufman believes that a physiatrist (a doctor who specializes in medical solutions to musculoskeletal disorders) is the best person for patients to see before consulting an orthopedic surgeon. As he says, when patients have chest pain, they don’t go to a cardiothoracic surgeon but a cardiologist.
Similarly, a patient with a headache would visit a neurologist rather than a neurosurgeon, while a person with stomach pain would go to a gastroenterologist before calling on a colorectal surgeon. Sandeep Kola, DO, is an interventional pain medicine physician serving patients at OrthoMed Pain & Sports Medicine in Sarasota and Venice, Florida. Thanks to his years of experience, Dr. Kola can treat patients effectively and efficiently so they can get back to living an active lifestyle.
He completed his doctoral degree at Kansas City University of Medicine and Biosciences in Kansas City, Missouri.
