Dr. Azeem completed his undergraduate education at James Madison University in Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident.
After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Falco.
Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida. He recently developed the first successful non-opioid based interventional pain clinic in the Sarasota, Florida, area.
His background helped shape Dr. Azeem’s approach to treating pain, and he includes both patients and their families in developing treatment plans so that everyone involved understands the origin of the patient’s pain as well as their available treatment options.
Currently, Dr. Azeem is an associate assistant professor in the pain/neurology department of the University of South Florida, and actively continues to conduct research and author peer-reviewed publications in addition to treating patients, President, Florida Society of Pain & Neuroscience, Chair of Strategic Development for State Society Engagement and Advocacy, American Society of Pain & Neuroscience and Board of Directors, Florida Prescription Drug Monitoring Foundation.

Maulik Bhalani, MD, is the founder and CEO of Florida Pain Medicine, an interventional spine and pain management facility with locations in Tampa, Wesley Chapel, Brandon, Zephyrhills, Riverview, Ormond Beach, and Port Orange, Florida. He is also the founder and medical director of Surgery Center of Florida. Dr. Bhalani is board-certified in interventional pain medicine and physical medicine & rehabilitation by the American Board of Physical Medicine and Rehabilitation and the American Board of Medical Specialties.
After earning his medical degree from Creighton University in Omaha, Nebraska, Dr. Bhalani went on to complete a physical medicine & rehabilitation residency at the University of South Florida, where he served as chief resident and completed a prestigious interventional pain medicine fellowship as well. Dr. Bhalani did his transitional medicine internship and partial anesthesiology training with the University of Maryland Medical Systems in Baltimore, Maryland.
Dr. Bhalani is the current Vice President of the Florida Society of Interventional Pain Physicians (FSIPP) and a member of several professional societies and regional organizations dedicated to pain medicine and physical medicine & rehabilitation. He currently serves as clinical associate professor and faculty for the pain medicine fellowship and Department of Neurology at the University of South Florida which allows Florida Pain Medicine to be a core rotation site for the ACGME Pain Medicine fellows and Physical Medicine & Rehabilitation residents.
With a strong belief that one should always keep learning, Dr. Bhalani continually pursues additional training and education in the latest interventional spine and pain management technology, attending pain conferences across the nation and bringing home the latest and most innovative practices for his patients. He continues to produce research and literature dedicated to pain management, and he participates in clinical trials for new pain treatments to help advance the field he loves.
When not helping his patients, Dr. Bhalani enjoys spending time with his family, playing and watching sports, and teaching residents and fellow surgeons at the University of South Florida. He is a huge sports fan: Go Creighton Bluejays, Tampa Bay Buccaneers, Tampa Bay Lightning, Tampa Bay Rays, and Orlando Magic.
Lane Peterson, D.O. offers an array of treatment options for numerous pain conditions at Florida Pain Medicine locations in Zephyrhills, Wesley Chapel, and Brandon, Florida. Dr. Peterson is double board-certified in Interventional Pain Medicine and Physical Medicine and Rehabilitation by the American Board of PM&R. Since 2008, he has specialized in the diagnosis and treatment of musculoskeletal and neurological pain of the head and face, spine, pelvis, and limbs. He performs interventional pain treatment procedures including epidurals, facet joint/medial branch blocks, radiofrequency ablation, sympathetic nerve blocks, neurostimulator implantations, and peripheral musculoskeletal and nerve blocks.
Dr. Peterson understands that each person who seeks treatment from him has a unique pain experience and history. He believes that a successful outcome starts with empathetic listening, thorough investigation, establishing an accurate diagnosis, and crafting an individualized plan for treatment. He utilizes a multi-factorial approach to treatment which aims to identify and address the underlying issues which led to the development of pain and to break the vicious circle of pain at as many points as possible.
Dr. Peterson graduated with honors from the University of Iowa with bachelor’s degrees in Biological Sciences and Physics. He then earned his medical degree at Des Moines University College of Osteopathic Medicine. Dr. Peterson credits his foundational training in Osteopathic Manipulative Medicine for developing his sensitive palpatory diagnostic skills which he uses to locate the source of pain.
Following medical school, Dr. Peterson lived in Miami, Florida for a year, where he completed a traditional osteopathic rotating internship at Westchester General Hospital. He completed his residency training in Physical Medicine and Rehabilitation at the Georgetown University Hospital/National Rehabilitation Hospital program in Washington, DC. While he was there, his training sites included the National Institutes of Health and Walter Reed Army Hospital, where he felt humbled to be able to care for soldiers returning from Iraq and Afghanistan.
Dr. Peterson completed his American Council of Graduate Medical Education-accredited Pain Medicine fellowship training at Virginia Commonwealth University Health System in Richmond, Virginia. Following this, he joined the teaching faculty as an assistant clinical professor in the Department of Physical Medicine and Rehabilitation, teaching fellows and residents while working at the Hunter Homes McGuire Veterans Administration Medical Center. He later practiced medicine in Overland Park, Kansas before recently relocating to Tampa.
In his spare time, Dr. Peterson enjoys spending time with his family, singing karaoke, and tinkering with electronics and computers.
Dr. Andrea Cordova-Ogg is board certified in Physical Medicine and Rehabilitation and is fellowship-trained in interventional pain management and musculoskeletal medicine. She completed her residency training in Physical Medicine and Rehabilitation at the Montefiore Medical Center/Albert Einstein College of Medicine in New York City. This was followed by extensive subspecialty training in Interventional Spine and Sports Medicine at the Orthopedic and Spine Specialists in Pennsylvania. She is highly skilled in the treatment of a variety of pain conditions using several distinct interventional procedures including ultrasound and fluoroscopy guided injections, radiofrequency neurotomy, neuromodulation, regenerative medicine, kyphoplasty/vertebroplasty and other state-of-the-art techniques.
Dr. Cordova-Ogg regularly participates in conferences and is a member of several professional societies including American Society of Physical Medicine and Rehabilitation, Spine Intervention Society, American Society of Interventional Pain Physicians, American Society of Regional Anesthesia and Pain Medicine, North American Neuromodulation Society and the North American Spine Society.
