Dr. Arfan Qureshi joined Revive Spine and Pain Center in 2022. As a Doctor of Osteopathic Medicine, his approach has always been holistic in nature. Dr. Qureshi has learned early on to treat the patient rather than the symptom.  The goal of treatment is to make the patient functional again using a multimodal approach of therapies, lifestyle modifications, medications, and interventions.

After graduating from Alabama College of Osteopathic Medicine, he went on to complete his residency training in Physical Medicine and Rehabilitation at Baylor Scott and White in Dallas, Texas. He then went on to subspecialize and completed an ACGME-accredited Interventional Pain Fellowship at University of Alabama Birmingham.

Dr. Qureshi is trained to perform a number of different fluoroscopic guided interventional pain management procedures. Dr. Selorm Takyi joined Revive Spine and Pain Center in 2020. He completed his physiatry residency at JFK Johnson Rehabilitation Institute, offered through the Department of Physical Medicine and Rehabilitation at Rutgers Robert Wood Johnson Medical School. With a passion for teaching, he served as an academic chief resident and graduated residency in 2019. He then completed his fellowship in regenerative orthopedics and musculoskeletal medicine.

Dr. Takyi practices compassionate medicine and was inducted into the Gold Humanism Honor Society in 2014 while attending Rutgers New Jersey Medical School. Specializing in both ultrasound-guided and fluoroscopic-guided techniques to treat pain and sports medicine injuries, he is excited to share his goal of using non-surgical procedures to help restore function and quality of life to his patients. With his training in physiatry, Dr. Takyi enjoys creating comprehensive treatment plans for his patients. Michael Rizkalla, MD, a skilled physician with double-board certification in sports medicine and physical medicine and rehabilitation, founded Revive Spine and Pain Center in 2017. Sanjay Shah, MD, is a pain management physician at SEPA Pain & Spine in Horsham, Langhorne, and Chalfont, Pennsylvania. He joined the team in 2006.

Dr. Shah earned his undergraduate degree from Johns Hopkins University in Baltimore, Maryland, and his medical degree from Ponce Health Sciences University in Puerto Rico. He then completed his residency in anesthesiology and a fellowship in pain management, both at Thomas Jefferson University Hospital.

He is a pain management physician who specializes in treating debilitating and painful conditions through various interventional orthopedic therapies. His philosophy for treating patients is based on staying current with the latest pain management, minimally invasive spine surgery, and regenerative techniques so he can provide the best possible care for his patients.

Jeremy Jaffe, MD, is a pain management physician at SEPA Pain & Spine in Horsham, Langhorne, and Chalfont, Pennsylvania. Dr. Jaffe joined the team in 1996. 

Since then, he has gained a reputation as a patient listener and effective communicator while providing a thoughtful, considerate, compassionate and comprehensive approach toward chronic pain management. Dr. Jaffe's areas of subspecialty interest include mechanical low back pain and opioid dependence counseling and treatment. He spent over 20 years helping to train medical students and anesthesia residents in his capacity as Director of Medical Education for United Anesthesia Services-Abington Division. Marlyn Alvarado, FNP-C, is a board-certified family nurse practitioner at Union Spine and Pain Management. Philip M. Rojo, DO, is a highly skilled nurse practitioner passionate about delivering customized care. Through his work at Union Spine and Pain Management, which has offices in Union, Scotch Plains, Bloomfield, South Bound Brook, New Brunswick, Old Bridge, and Woodbridge, New Jersey, Philip helps patients feel more comfortable as they progress in treatment. 

A lifelong learner, Philip earned a Bachelor of Science in nursing from Rutgers, The State University of New Jersey, in New Brunswick. He remains at Rutgers to complete a Master of Science in nursing, focusing on adult and geriatric care.
