A native to Fernandina Beach, FL, Kimberly left to attend the University of Florida where she obtained a BS in Exercise Sports Science with a specialty in Athletic Training in 2006. During that time, Kim had the opportunity to work with area high schools, the intramural program, and the UF Volleyball team. Kimberly was even able to accompany the UF Volleyball team to a SEC championship and NCAA tournament! After UF, she attended the University of St. Augustine to obtain a doctorate in physical therapy, where she graduated in 2009. Kimberly also achieved a certification in manual therapy in 2010, and a certification to practice dry needling in 2021. Kim has practiced in traditional and non-traditional rehabilitation settings and has found that a one-on-one approach with a focus on manual therapy, or hands on treatment, benefits the client best.
Dr. Holmes is the owner of Holmes Physical Therapy, LLC. He has a passion for helping friends and fellow West Tennesseans feel better and move better with the goal of staying better. He cares for each of his patients by listening to their problem as he has been a patient who suffered from joint pain himself.
The path to Holmes Physical Therapy, LLC, started when Ken Keach, PT, DPT, Cert MDT, invested his time in Dr. Holmes as a young PT student. Dr. Keach was Dr. Holmes’s clinical instructor and challenged him to question every move a patient made and to maintain integrity in every aspect of patient care. Dr. Keach’s investment taught Dr. Holmes how to resolve his very own severe case of neck pain, which he was previously unable to resolve and had been worsened by other therapy approaches. This opened Dr. Holmes’s eyes to the fact that treating joint pain can be very simple at times and is on a mission to help other find relief and teach them how to treat themselves.
“Scott is the best. He is very knowledgeable in several different PT techniques and employments all of them to build the best possible solution for each patient. He kept me from having rotator cuff surgery by just showing me how to work out the scar tissue. He has taught me how to walk differently to help with my lower back pain. Not to mention the stretching to help with my neck pain. The list goes on and on. 20+ years of railroading has taken a toll on my body, as it does to anyone working.”

Dr. Darius Porter (he/him) grew up his whole life as a Tacoma, WA native, and graduated from Foss High School. He then graduated with his Bachelor’s degree in Kinesiology from Washington State University (Go Cougs!). While waiting to get accepted into a physical therapy program, Darius worked as a Physical Therapy Aide to build his knowledge base and gain experience. He was then accepted and completed his Doctor of Physical Therapy program from Hampton University in Virginia. During his education he has won the Bruya-Wood Conference Award for Critical Thinking and making it on the Dean’s list.
Darius’ passion for helping others comes from his own personal experience of inadequate care as a youth and wants his patient’s to not only know the benefits that Physical Therapy can provide, but also how to prevent future injuries later in life.
His hobbies include bowling, playing basketball, playing pool, golfing, attending sporting events and concerts.
“I've had quite a few Massage therapist in my time. One I would swear by and never leave if I had a choice.. until I met Suzanne, she has been beyond amazing in my healing process and has literally made me forget all about the others from before. She has a deep and intense Yoga knowledge and training which makes her almost supernatural in the healing experience. At the start of every session, with little knowledge of what maybe affecting you, instantly when she touches you she can read the energy of the issue and draws right to it. Her Yoga knowledge really knows how to pin point what it is and how to suppress the pain and cramping while relaxing your negative energy. I've never and I mean never experienced anything like Suzanne and her skills before. Your team should be proud you have found a true diamond in the rough. Just amazing, hands down the best ever. Thank you for that.”
I will always lead with the assumption that the most important person in the treatment room or gym, is not the provider, but the patient. I want to know about you; your interests, and your goals. After that comes a brief background on myself. I graduated from Lyndon Institute in Lyndonville Vermont, where I currently live with my family. I graduated with honors from the University of Vermont and was Captain of the Division 1 track and field program. I took a little time off to scour the medical landscape before accepting the call to be a Physical Therapist and graduated from UVM with a Doctorate in Physical Therapy. After cutting my teeth at an amazing outpatient clinic in St. Johnsbury Vermont for 9 years, I once again followed the call, and opened up Strong Oaks Physical Therapy. Every patient has a story and a journey. I will scour the whole kinetic chain to help clear all the physical barriers on that path. Good health does not END with Rehab, it STARTS with optimization!
“Very good about getting someone in as quick as possible. Very caring and great support to patient. Recommend this facility to everyone. Keep up the good work!”
